To be executed before a Court of Record, or some officer thereof having cusody of its seal, State of Louisiana County of Vernon On This 28 day of August, A.D., one thousand eight hundred and ninety, personally appeared before me J.M. Jackson J.P. of the 6th ward V? Court, the same being a Court of Record within and for the County and State aforesaid Anna Mc Cullough, aged 80 years, who, being duly sworn according to law, declares that she is the widow of Isaac Mc Cullough who enlisted under the name of Isaac Mc Cullough, at Alexandria La on the 22nd day of Apr, A.D. 1914, in Co B 1 Regt La Scouts and served at least ninety days in the late War of the Rebellion, and who was honorably discharged on the (blank) day of (blank) 186(); and died on the (blank) day of Sept 1864, at Morgansey La. That she was married under the name of Anna Greyer to said Isaac Mc Cullough on the 20 day of January 1829 by Squire Franklin in Pike Co. Miss there being no legal barrier to said marriage Isaac Mc Cullough That she has not remarried since the death of the said Isaac Mc Cullough That she is without other means of support than her daily labor. The names and dates of all the children now living under sixteen years of age of the soldier by herself are as follows: James B. born Oct 16th 1849; Zimmarman born Sept 10th 1852; Malinda-ann born Feb 25 1865 That she has (blank) heretofore applied for pension, and the number of her former application is 415081. That she makes this declaration for the purpose of being placed on the pension roll of the United States, under the provisions of the Act of June, 1890. She hereby agrees to pay her attorneys L.C. Wood & Co. the full legal fee of $1. She hereby appoints L.C. Wood & Co., U.S. Pension Attorneys, 512 F Street, N.W., Washington D.C., her true and lawful attorneys, to prosecute her claim.
