This Affidavit should be in the handwriting of the Physician whose statement should conform as near as possible to the marginal insturctions hereonwhich he should read with great care.The diagnosis of the soldiers condition should be so full and compelete that a medical man will be enabled at once unmistakeably to recognize the character of the disease,wound or injuries.The statement should be in Narrative Form and the marginsl instructions should not be referred to by number in the affidavet. Jones clerk Distrect Court in and forsaid county and state,Dr.C R Wilcox of Gansville county of winn state of Louisiana who I hereby certify is a respectable and credible person,and who being duly sworn,declares in relation to the aforesaid claim that his age is forty years;that he has been a practising physician for the past fifteen years;that he has made careful medical examination of the above-named claimant and finds his present physical condition as follows A suffers from muscular rhumatism of the back disabling him from such manual labor as requires him to bend or stoop.This disability is rated one fourth he also has a direct inguinal hernia of the left side which requires the constant use of a truss rated at one half.
