I hereby certify that in compliance with the requirements of the law I carefully examined this applicant, who states that he is suffering from the following disability, incurred in the service, viz: Rupture If a pensioner, fill in the amount; Ifnot, erase the whole line (line drawn through). He makes the following statement upon which he bases his claim for original The Claimant say he was in the military service and was thrown from a horse near morganza La and was struck by the horses fore foot in the left ingunal region which was followed by rupture in about 6 months and he belives that the cause of his rupture Upon examination we find the following objective conditions;Pulse rate,72 respiration 20 temperature,98 3/5 height 6 feet 1/2 inches;weight,136 pounds;age,70 years And upon examination ofthe claimant I find a reducuble ingunal hernia (left side) decending through the external abdamnalring into the scrotum there is no strangulation or adhesious at either of the rings nor any other conflicutrous I find senile dibility.Judging from claimant age,ffebleness and his hernia I think he is about 3/4 incupucitated to do manual lobor about 1/4 from the hernia and about 1/2 from old age I find no evidencs of disability attributable to vicious practices He is in my opinion entitled to a fourth (1/4) rating for disability caused by hernia one half(1/2) for that caused by old age D.H Caldwell M.D.
