In pension claims as Lettica - This first is taken as correct name. P. B. WAR OF 1812 CLAIM OF WIDOW FOR SERVICE PENSION Under the provisions of Sections 4736 to 4740, inclusive, Revised Statutes, and the Act of March 9, 1878 N. B.-All the blank spaces in this form must be carefully filled up in accordance with the instructions on the back hereof, and from the best information possessed, or obtainable, by the applicant. State of Mass, County of Hampden, ss. On this 7th day of June, A.D. one thousand eight hundred and Seventy Eight, personally appeared before me Register of Probate Court, the name being a court of record within and for the County and State aforesaid, (1) Lettica Davis, aged 78 years, a resident of Springfield, in the State of Massachusetts, who, being duly sworn according to law, declares that she is the widow of (2) Austin Davis deceased, who was the identical (3) Austin Davis, who served under the name of (4) Austin Davis as a (5) Private in the company commanded by Captain Erastus Strong in the regiment of Conn militia, commanded by Col. Ezra Brainerd in the war of (6) 1812; that her said husband (7) Enlisted at Ellington, Conn on or about the 18th day of August A.D. 1814 for the term of --, and continued in actual service in said war for the term of (8) two months & 11 days and whose services terminated, by reason of (9) -- at -- on the 28th day of October A.D. 1814 She further states that the following is a full description of her said husband at the time of his enlistment, viz: (1) height about 5 feet 8 inches - dark Complexion, Brown Hair black eyes and by occupation a farmer. She further states that she was married to the said Austin Davis, at the city (or town) of Becket, in the county of Berkshire, and in the State of Mass. on the 16th day of October A.D. 1816 by one (11) Rev. J. L. Mills, who was a (12) Minister of the Gospel; and that her name before her said marriage was Lettica Snow; and she further states that (13) Neither She or her husband had been previously married and that she has not since remarried and that her said husband (14) Austin Davis, died at Becket, in the State of Mass, on the 25th day of September A.D. 1850; and she further declares that the following have been the places of residence of herself and her said husband since the date of his discharge from the Army, viz: (15) in Becket Mass and Springfield, Mass. She makes this declaration for the purpose of obtaining the Pension to which she may be entitled under Sections 4736 to 4740, inclusive, Revised Statutes, and the Act of March 9, 1878, and hereby appoints 26. M. Coney of Springfield Mass, her true and lawful attorney to prosecute her claim. And she further declares that she has heretofore made no application for (16) Pension but has received a land Warrant but the number and date she has no recollection of but refers to it for such facts as she is unable to recall and that her residence is No. 85 Elliott street, city (or town) of Springfield, county of Hampden State of Mass, and that her post office address is 85 Elliott St Springfield Mass /s/ Lettica Davis (Two attesting witnesses who can write their names.) /s/ John N. Norton /s/ Almon White Also personally appeared John N. Norton, aged 42 years, residing at No. 431 Hancock street in Springfield, and Almon White, ages 53 years, residing at No. 322 Union street in Springfield, persons whom I certify to be respectable and entitled to credit and who, being by me duly sworn, say that they have known the said Lettica Davis for 23 years and for 40 years, respectively; that they were present and saw her sign her name (or make her mark) to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and their acquaintance with her, that she is the identical person she represents herself to be; and they further say that they are able to identify her as the person who was the wife of the identical (17) Austin Davis, who rendered the service alleged in the above application (in the company of Captain Erastus Strong, in the regiment of Col. E. Brainard of Conn Militia in the war of 1812) by the following named facts and circumstances, viz: (18) said Norton knows claimant to be a widow: the said White well remembers said Austin Davis the husband of Claimant & knew they lived together as man & wife and that said Austin Davis had deceased and both certify that she has not remarried /s/ John N. Norton /s/ Almon White Sworn to and subscribed before me this 7th day of June, A.D. 1878; and I hereby certify that the contents of the above declaration, &c, were fully made known and expained to the applicant and witnesses before swearing, including the words - erased, and the words - added; and that I have no interest direct or indirect, in the prosecution of this claim. /s/ Saul B S??????
