Enlisted: Boston, Mass., December 5, 1917.
Died: U. S. A. C T. Ticonderoga, September 30, 1918.
Cause: Died when vessel was torpedoed by enemy.
 Next of kin: Mother, Emma Chaput, 79 Central Street, Ipswich, Mass.
