Died: On the U. S. S. Tampa when that vessel was sunk. 
Date: September 26, 1918. 
Cause: Enemy action. 
Next of kin: Minnie Hoffman (mother), 218 Bremen Street, East Boston,
  Mass. 
Appointed from Massachusetts.
