Elaine Joan (Glaza) Van Mullekom
March 7, 1936 - February 3, 2004
Van Mullekom, Elaine J. Bay City, Michigan.  Mrs. Van Mullekom passed away 
Tuesday, February 3, 2004 at Bay Regional Medical Center, age 67 years.  The 
daughter of the late Theodore and Mary (Zajac) Glaza was born March 7, 1936 in 
Bay City.  

She is survived by her children: Laurie Blake of Vista, California, John Black 
of Santa Monica, California and Jennifer Van Mullekom of Oceanside, California; 
her granddaughter, Ashleigh Blake of Vista, California; three brothers: Richard 
Glaza, John (Pam) Glaza and Bill (Nancy) Glaza; one brother-in-law, Ray 
Malizsewski; several nieces nephews and cousins.  In addition to her parents she 
was also predeceased by her husband, Paul Van Mullekom; her sister, Nancy 
Maliszewski; two brothers, Robert James and Thomas M. Glaza. 

At the request of Mrs. Van Mullekom Cremation has taken place.  The Family will 
be present at the Gephart Funeral Home, Inc. on Saturday, February 7, 2004 from 
10:30 a.m. and until 12:00 o'clock noon to greet relatives and friends with 
inurnment following in Floral Gardens Cemetery.  Memorial Envelopes for the 
American Heart Association will be available at the funeral home.  Gephart
