Kozachik, M.D., F.A.A.F.P., 65, of Portland died May 10, 1992, in Portland. He was born in Oxford on July 27, 1926, the son of Stephen and Wanda (Kujawa) Kozachik. He attended Oxford High School; pre-medical school at Michigan State University; and received his medical degree from the University of Michigan in 1955. He wrote his Oxford High School song. He was a veteran of World War II, a Portland family physician since 1956, and was awarded and became a Fellow in the American Academy of Family Physicians in 1990. He was the Portland High School football team’s doctor for 35 years; past president of the Portland Band Boosters; served as band physician for band trips and master of ceremonies for numerous band functions; a member of the Portland Board of Education from 1961 through 1972; Portland Jaycee president in the early 60s; active in the Portland Civic Players; member of the Michigan State Alumni Band; member of the Lansing Concert Band; member of the Knights of Columbus; and member of St. Patrick’s Catholic Church in Portland, serving as a lector, musician, and participating in the church’s religious education program. He is survived by wife, Mary Ann; three daughters, Jean Kozachik, M.D., of Traverse City, Carol (Michael) Delaney of Belmont, and Eileen Kozachik of Grand Ledge; six sons, Peter Kozachik of San Francisco, Calif.; Stephen (Ann) Kozachik of Tucson, Ariz., Kenneth (Barbara) Kozachik of Kentwood, Thomas (Sharon) Kozachik of Portland, David Kozachik of Wyoming, and Phillip (Gerri Lynn) Kozachik of Lansing; six grandchildren; brother, Stephen Kozachik of Sunfield; and sister, Mary Ann (Jack) Delling of Flushing. Vigil services were recited by the Rev. Peter Nickels, O.F.M.,CONV. On May 12 at the Gorsline-Runciman Neller Chapel in Portland. Mass of the Christian Burial was held May 13 at St. Patrick’s Catholic Church in Portland, with the Rev. Peter Nickels and concelebrants the Rev. Julian Reginato and the Rev. Mark Przybysz of St. Patrick’s Catholic Church, and Sebastian Cunningham, O,F.M. CONV. Casket bearers were Marvin Thrasher, Richard Willemin, Jack Delling, Michael Delaney, Kenneth Bloomquist, and John Nickels. Interment in Danby Cemetery.
