Ionia Sentinel-Standard - Sept., 2012

Thomas John Kozachik, 51, of Ionia, passed away unexpectedly Sept. 14, 2012. He 
was born Nov. 14, 1960 in Lansing, he son of Dr. Martin J. and Mary Ann 
(Nickels) Kozachik.

He graduated from Portland High School in 1978 then went on to graduate from 
Western Michigan University in 1983. Thomas loved to hunt and fish and was an 
avid fan of MSU. He worked for the Michigan Department of Corrections in Ionia 
for 26 years and retired in December 2011.

Thomas is survived by brothers and sisters, Dr. Jean Kozachik of Traverse City, 
Kenneth (Barbara) Kozachik of Kentwood, Carol Delaney of Grand Rapids, David 
Kozachik of Hudsonville, Phillip Kozachik of Traverse City, and Eileen Cannon 
of Portland, as well as many nieces and nephews.

He is preceded in death by his parents and sister-in-law Gerri Lynn Kozachik.

Memorial services will be held Wednesday, Sept. 26 at 7 p.m. at Lake Funeral 
Home in Ionia. The family will receive friends and relatives Wednesday 2 to 4 
p.m. and 6 to 7 p.m. at the funeral home.
