Note:  Death certificate shows that Jay Allen Henry was the son of Allen Henry 
and Mariah Woodworth (both of NY), male, white, single, farmer, born Dec 3, 
1855 NY (headstone shows Dec 3, 1854), died July 14, 1913 Evart, Osceola Co., 
MI at the age of 58 years 11 months and 7 days, cause of death cerebral 
apoplexy.  Informant was Mary E. Henry of Evart, Michigan.  Burial was on July 
19, 1913 Striker Cemetery.
