Hines, duly authorized to administer oaths within and for the County and State aforesaid Henry Kelsey age 61 years, a resident of the village of Olivia, County of Renville, State of Minnesota, who, being duly Sworn according to law, declares that he is the identical Henry Kelsey who was first enrolled on the 26th day of June 1861 in Company B, Second Regiment Minnesota Vols in the War of the Rebellion, and served at least ninety days, and was honorably discharged from final service at Chattanooga, Tennessee on the 26th day of June 1864. That he is wholly unable to earn a support by reason of stomach trouble, rheumatism, lumbago, heart trouble, deafness, bad eyesight, broken ankle and paralysis of right wrist. That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent. In addition to the above he had _____ military or naval service in any army or navy ______ prior to thereto. That he has applied for pension under application No. 928.403. His ankle and thereby causing permanent injury. That about six weeks after said injuries he suffered the paralysis of his right wrist and arm, thereby incapacitating him for manual labor of all kinds. That his other disabilities, as stated in application for pension in June 1890 were incurred during his service in the army from 1861 to 1864. Meaker M.D. aged 26 years, a resident of Olivia in the County of Renville and State of Minnesota, whose Post-Office address is Olivia Minnesota, who, being duly sworn, delcares in relation to the aforesaid claim as follows: I gained a knowledge of Mr. H. Kelsey’s case being the attending physician. Mr. H. Kelsey sustained a full fracture of the right leg, in which the ankel (sic) was also severely sprained which well entail a permanent (sic) injury. Being afflicted with rheumatism in ? years, such an injury as this is ? aggrivated (sic) the rheumatic trouble becoming localylized (sic). He has further trouble with his arm the right arm due to an effection (sic) of the musclo-spinal nerve, a trouble which is not resent (sic) but preasantly (sic) is greatly aggrevated (sic) amounting to paralysis of the arm and ahnd and he has (no) use of his hand incapasetating (sic) him for manual labor; ? rheumatic. B 2nd Minn Vol. Inf in the service of the United States, in the Civil War, and was HONORABLY DISCHARGED at Chattanooga, Ten, on the 26 day of June, 1864. That he also served (blank). That he was not employed in the military or naval service of the United States otherwise than as stated above. That she is the widow of Henry Kelsey, who was enrolled (enlisted) under the name of Henry Kelsey at Wabasha on the 26th day of June 1861, and honorably discharged June 24, 1864, having served ninety days or more during the late war of rebellion. That the soldier was not in the military or naval service of the United States except as stated above. That she was married under the name of Lenore H. McMillen to said soldier at Wabasha on the 31st day of April 1865 by Rev. Whorton; that there was no legal barrier to the marriage; that she had not been previously married; that the soldier had not been previously married. That the said soldier died June 21st 1907 at Woodborn, Oregon that she was not divorced from him; that she has not remarried since his death; and that she is without other means of support than her daily labor and an actual net income not exceeding two hundred and fifty dollars per year. That the said officer left the following-named children who are now living and under sixteen years of age, to wit: (none listed) That she has not heretofore applied for pension.
