Rucker, Notary Public within and for the County and State aforesaid, Wm. S. Evans, aged 48 years, a resident of the county of Chariton, State of Missouri who, being duly sworn according to law, declares that he is the identical William S. Evans who was enrolled on the [blank] day of [blank], 1864, as a private, Company "C" 135th Ind. Vol. in the war of the Rebellion, and served at least ninety days and was honorably discharged at Indianapolis, on the [blank] day of [blank], 1864. That he is totally unable to earn a support by reason of loss of use of left leg caused from protracted spell of fever, causing leg to perish away. That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent; that he has never applied for pension under application no. [blank]. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of June 27, 1890. Claimant's Testimony State of Missouri, County of Chariton In the matter of the application for William S. Evans, Pension No. 955,955 of Co. "C" 135th Regt. Ind. Vols. Inf; personally comes the claimant, who, being first sworn on oath, says that he is the identical William S. Evans, who is a claimant for pension on account of injuries received prior his discharge from said service, and that his claim for pension is filed as number 955,955. That he served as a private of Co. "G." 55th Regt. Ind. Vol. in which company he enlisted about June 1862 and was honorably discharged about October 1862, and that he next served as private of Co. "C." 135th Regt. Ind. Vol (hundred day men) having enlisted in the latter about May 1864 and was honorably discharged about November 9th 1864, and that he has not since his said discharge last stated, served in any other military or naval service of the United States of America. Evans for the past 40 years and have been his physician for a no. of years and at this time his condition is such that he is hardly able to take care of himself. There is a general disability ----, a paresis of bowels, femoral rupture and a prostatic trouble, vertigo, and has not been able most of the time since having an attack of the flu 2 years ago to take care of himself. To take in consideration all of his disabilities and age he is in a very serious condition. A.C. Evans enlisted May 1862 at Indianapolis, under the name of William S. Evans, in Co. G 55th Ind. Vol. Inft. and was honorably discharged Autumn, 1862, at Indianapolis, having served the United States in the Civil War. That he also served in Co. C. 135th Ind. Vol. Inft. from May 1864 until Autumn of 1864. That otherwise than herein stated he was not employed in the United States military or naval service. That his personal description at time of first enlistment was as follows: Height 5 feet 10 inches, complexion fair, color of eyes grey-blue, color of hair light brown, that his occupation was teacher. That since leaving the service he has resided at Keytesville, Mo., and Kingman, Kansas and his occupation has been clerical. That he requires the regular personal aid and attendance of another person and has required such aid and attendance since 1921 on account of the following disabilities: chronic bronchitis, habitual constipation, rheumatism. So nearly blind and so nearly helpless as to require daily aid. That he did not service in the Army, Navy, or Marine Corps of the United States between April 6, 1917, and February 9, 1922, or at any time during said period. Dr. Pope of Kingman was next door neighbor and friend and physician for the last 8 years. 25. State the names of the persons by whom the pensioner was nursed during the last illness. Has there been paid, or will application be made for payment to you or any other person, any part of the expenses of the pensioner's last sickness and burial by any State, county, or municipal corporation? No. The Gov't sent $100.00 as burial expenses. Each charge entered below should be supported by an itemized bill of the person who rendered the service or furnished any supplies for which reimbursement is demanded and should show, over his signature, by whom paid, or who is held responsible for payment, and contain the name of the pensioner for whom the expense was incurred or service rendered.
