AFFIDAVIT TO PROCURE EXAMINATION OF AN INSANE PERSON

STATE OF NORTH CAROLINA, Jones COUNTY

THE UNDERSIGNED, residing in said county, makes oath that he has carefully 
examined Jane Bryan, the alleged lunatic, and believes her be an insane 
person, and to be, in the opinion of the undersigned, a fit subject for 
admission into an Insane Asylum.

Dated 4th day of April, 1887

Subscribed and sworn to before me, (signed) E. R. Page JUSTICE OF THE PEACE

N.B. – upon the coming of such alleged insane person before such Justice, as 
provided in Sec.17 of the Law to incorporate the Insane Asylums of the State, 
Ratified 20th February, 1883, if the Justices shall so determine, they shall 
render the following judgment of committal.  (Every part of the following 
forms is required by law to be filled.)

STATE OF NORTH CAROLINA, Jones COUNTY

Upon examination of R. A. Whitaker, M.D., Abner Dawson and Amelia Mills, we 
decide that Jane Bryan is insane and no friend becoming bound, as the law 
directs, we adjudge and direct that said Jane Bryan be removed to the EASTERN 
NORTH CAROLINA INSANE ASYLUM, as a patient, and that a warrant issue 
accordingly.

THE STATE OF NORTH CAROLINA

To the Sheriff or Constable of Jones County Greeting

WHEREA, It has been made to satisfactorily appear to us E.R. Page and I. S. 
Smith, Justices of the Peace for said county, that Jane Bryan, citizen of the 
State is an insane person, that ____ has a legal settlement in said county, 
and is a fit subject for an Insane Asylum, and that she being at large is 
injurious to herself and disadvantageous, if not dangerous to the community:  
You are hereby commanded to take said Jane Bryan and convey her to the Eastern 
North Carolina Insane Asylum, and there deliver her to the Superintendent 
thereof for safe keeping.

Given under my hand this 4th day of April 1887

ratified February 20, 1883, AND SHOULD BE FILLED UP IN EVERY APPLICATION.

	ANSWER has been married but not at time

QUESTION 5.  What is the supposed cause of insanity?
	ANSWER  Her mind has been gradually growing weak for some time

QUESTION 6.  In what way is the disease exhibited?
	ANSWER  no interest in anything, mind wandering she goes from house to 
house for no reason

QUESTION 7.  Has any medical treatment been pursued?  If so, what kind and by 
whom?
	ANSWER   none 

QUESTION 8.  How long has she been insane? Count from first symptoms, and give 
all known symptoms from   that time to this date.
	ANSWER    she has been insane for about 3 years but growing worse all 
the time

QUESTION 9.  Has the patient manifested any propensity to injure herself or 
others?  If so, what way, and how often?
	ANSWER   She went to Ms. Kelly House and damaged some uneasiness by 
taking the baby, but made 
To stop it.

QUESTION 10.  Has she been subject to epilepsy?
	ANSWER  not known

QUESTION 11.  Has any of her ancestors been insane?  If so, state what 
ancestors and what was the character of their insanity?
	ANSWER  not known

QUESTION 12.  Has she any family, and if so, what persons compose it?
	ANSWER  3 children, one boy 2 daughters

QUESTION 13.  Are any of them insane, and what is the character of such 
insanity?
	ANSWER  do not know

QUESTION 14.	  What is the occupation of the patient?
	ANSWER  she’s a wanderer

QUESTION 15.	  How many attacks of mental disease has the patient had?
	ANSWER  do not know

QUESTION 16.	  Are the parents of the insane person related by blood?  If 
so, what is the degree of relationship?
	ANSWER  do not know 

QUESTION 17.  Has the Patient property?  If so, state in what such property 
consists, an what is the value thereof.
	ANSWER  No

QUESTION 18.  Is she under any forcible restraint?  If so, what?
	ANSWER  She is in jail  

QUESTION 19. Has the patient received any aid from the County?  If so, what?
	ANSWER  she is cared for by the jailer

QUESTION 20.	  Give NAME and post office of the nearest relative or friend 
of the patient with whom the Superintendent of the EASTERN NORTH CAROLINA 
INSANE ASYLUM can correspond, as circumstances require, for the benefit of the 
patient.

	ANSWER  Ambasik Cobb, Post Office Kinston , NC 

QUESTION 21.  Give any information in your possession, not embraced in the 
above questions, which may throw light on the mental or physical condition of 
the patient?

TO THE EXAMINING PHYSICIAN.- Replies to the following questions, though not 
required by law, will enable the Superintendent of the Asylum to form a more 
correct opinion of the case, as well as aid in the treatment.

QUESTION 1.  Are any of her relatives insane, deaf, dumb, blind, idiotic, 
epileptic or paralyzed?  If so, state relationship.

	ANSWER  Not known

QUESTION 2.  If patient be a married woman, state maiden name.
	ANSWER  Jane Jackson

QUESTION 3.	Has she any education or religious bias?  If so what?
	ANSWER  Baptist

QUESTION 4.  Where was she born?
	ANSWER  Not known

QUESTION 5.  Has she any delusions?  If so, what are they?
	ANSWER  Depression

QUESTION 6.  Is she destructive to clothing or furniture?
	ANSWER  No

QUESTION 7.  Is she filthy or indecent?
	ANSWER  No

QUESTION 8.  What is her bodily condition?  (wounds, bruises, rupture, 
pregnancy, etc.)?
	ANSWER  fine

QUESTION 9.  Is she in jail?  ANSWER  yes

oath that the foregoing answers are true t the best of their knowledge and 
belief.
