Each record is a certification of lunacy on a pre-printed form. There is also a 
certification for discharge if the person is found to no longer be a danger to 
self or others. The following is the standard wording:

	BE IT REMEMBERED, That on the ___ day of ___, 19__, personally appeared 
_____________, who made affidavit upon oath before me, that __ had carefully 
examined _____________, the alleged lunatic, and believes ____ to be an insane 
person, and, in the opinion of affiant, a fit subject for admission into a 
hospital for the insane.
	Thereupon I caused the said ______________to be brought before me, and 
upon examination of ___________, a licensed and reputable physician, a resident 
of this state, and ___________, a member of the alleged lunatic’s family, and 
also ______________________ and upon examination of the alleged insane person, 
and the hearing of the testimony, I did decide that ___________ is _____ insane, 
(and ______ was by me discharged), and it appearing that the said lunatic has a 
legal settlement in ___________ County, and that ____ being at large is injurious 
to ___self and disadvantageous if not dangerous to the community, and no friend 
becoming bound as the law directs, I did adjudge and direct that _______________ 
be committed to the State Hospital at _____________, N.C., as a patient.
	Thereupon, I issued my warrant of commitment to the Sheriff of _________ 
County, commanding him to take ___________ and convey ____ to the proper 
hospital, and there deliver ___ to the Superintendent thereof for safe keeping. 
	And of this Inquisition of Lunacy, I this day transmitted a copy to the 
Superintendent of THE STATE HOSPITAL, at ____________, N.C., together with the 
examination of witnesses.
