1. Lewis's commission from governor of Connecticut as militia major, 6th Regt. 
of Volunteers, mustered in at Wilmington, N.C., Mar. 7, 1865; 

2. Commission by President McKinley as additional paymaster of volunteers (1898);

3. Certificate of membership in the Loyal Legion (1902).
