STATE OF Nebraska County of Richardson ss. On THIS 17th day of July A.D. one thousand eight hundred and ninety, personally appeared before me J. L. Dalbey of Stella, Nebraska a Notary Public within and for the county and State aforesaid Jacob Pritts aged 48 years, who, bing duly swaorn according to law, declares that he is the identical Jacob Pritts who was ENROLLED as a private on the 12 day of August 1862 in Company 'C' of the 142 Regiment of Pennsylvania Vol Infanty commanded by John Boyds and was honorably DISCHARGED at Baltimore, Md on the 24th day of August, 1865; that his personal description is as follows: age 48 years; height 5 feet 11 inches; complexion dark; hair light; eyes grey. That he is now suffering from a physical disability of a permanent character, not the result of his own vicious habits, which incapacitates him from the performance of manual labor in such a degree as to render him unable to earn a support from Kidney complaint as general disability. He therefore makes this declaration for the purpose of being placed on the dependent pension roll of the United States. He hereby appoints, with full power of substitution and revocation, Hubbard & Case of Albany, Mo., his true and lawful attorney to prosecute his claim. That he has received a pension No 114.822; that his residence in No. (blank)_______ street (blank)_______ and that his postoffice address is Stella, Nebraska. (signed) Chas. Derstine residing at Stella, Nebraska and S. L. Mains residing at Stella, Nebraska persons whom I certify to be respectable and entitled to credit, and who, being by me duly sworn, say that they were present and saw Jacob Pritts the claimant sign his name (make his mark) to the foregoing declatration; that they have every reason to believe from the appearance of said claimant and their acquaintance with him, that he is the identical person he represents himself to be; and that they have no interest in the prosecution of this claim.
