Cavalry, (Harris' Light Brigade, Regiment New York State Volunteers was enlisted by Capt. Weed the ~ Regiment of ~ at Plattsburgh, New York on the Eighteenth day of Sep ~ tember,1861 to serve three years; he was born in Acworth, in the State of New Hampshire, he is Forty One years of age, Five feet Four inches high, Dark comp ~ lexion, Dark Eyes, Grey Hair, and by occupation when enlisted a Teamster. During the last two months said solider has been unfit for duty ~ days. Admitted into General Hospital, in ALexandria, Virginia, on October 13th, 1862. * Horse fell on him. One Thousand, Eight Hundred, and Eighty, personally appeared before me, Dep. Clerk of the County Court, a court of record within and for the County and State aforsaid, Charles Smith, aged 61 years, a resident of the Town of Jay, county of Essex, State of New York, who, being dully sworn according to law, declares the he is the identical Charles Smith who ENROLLED on the ~ day of October 1861, in Company M of the 2nd Regiment of the New York Cavalry commanded by: Captain ~ ~, and was honorably dischared: that his personal description is as follows: Age, 61,years, height, ~ feet, ~ inches; complextion ~ ; hair:~; eyes:~; That while a member of the prganization aforesaid, in the service and in the line of his duty at *Manassas Junction in the Second Battle of Bull Run, in the State of Virginia on or about the 30th day of August, 1862, he was thrown from his horse causing injuries to his head, back, and his kidneys. ------------------------------------------------------------------------------------ --- ------------------------------------------------------------------------------------ --- ------------------------------------------------------------------------------------ --- That he was treated in the hospitals as follows: In Regt, Hospitaal aaaaaat Halls Hill, Virginia by Surgeon Alkey, for about three weeks, then, sent to ~ Hosptial at Alexanderia, Virginia where he remained till he was discharged.---------------------------------------- ------------------------------------------------------------------------------------ ------ ------------------------------------------------------------------------------------ ------ That he has NOT been employed in the military or naval service otherwise than as stated above. stated above. ---------------------------------------------------------------------------- ------------------------------------------------------------------------------------ ------ This since leaving for service this applicant has resided in the Town of Jay, New York State. ------------------------------------------------------------------------------------ ------- and his occupation has been that of a Farmer.---------------------------------------------- ------------------------------------------------------------------------------------ ------- That prior to his entry into the service aboved named he was a man of "good, sound, physical health, being when enrolled a Farmer That he is now partially disabled from obtaining his sub~ sistence by manual labor by reason of his injuries, above describes, receieved in service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States. He hereby appoints, with full power of subsitution and revocation, Henry A. Taylor, of Moores, State of New York, his true and lawful attorney to persecute his claim.
