Riel aged 55 years, a resident of the town of Dennisville county of Cape May, State of New Jersey, who, being duly sworn according to law, declares that he is the identical Charles P. Riel who was ENROLLED on the first day of September, 1862, in Co. "F" 25 N.J. Vols as Private(Here state rank, company and regiment in Military service, or vessel, if in the Navy), in the war of the rebellion, and served at least ninety days, and was HONORABLY DISCHARGED at Beverly, on the Twentieth day of June, 1863. That he is partly unable to earn a support by reason of physical infirmity and disease. That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent. Riel aged 55 years, a resident of the town of Dennisville county of Cape May State of New Jersey who, being duly sworn according to law, declares that he is the identical Charles P. Riel who was ENROLLED on the 1st day of Sept. 1862 in Company F of the 25 Regiment cf N.J. as a private commanded by ____________ and was honorably DISCHARGED at Beverly, N.J. on the 20th day of June 1863. Height 5 feet 9 1/4 inches dark complexion, color of hair Grey. That while a member of the organization aforesaid, in the service and in the line of his duty at Battle of Fredericksburg in the State of Virginia on or about the 20th day of Dec. 1863, he contracted a kidney trouble, which resulted in gravel, and he has suffered from the same and the effects of more or less ever since. That he was treated in the hospitals as follows: was treated by Dr. Bateman for what he called Jaundice. (Here state the names or numbers, and the localities of all hospitals in which treated and the dates of entering and leaving each, as now remembered.) That he has not been employed in the military or naval service otherwise than as stated above (If in other service, here state what it was, with Co., Reg't, rank and date enlisted and discharged.) That since leaving the service this applicant has resided in the county of Cape May in the State of N.J. and his occupation has been that of a farmer. That prior to this entry into the service above named he was a man of good, sound, physical health, being when enrolled a Sawyer. That he is greatly disabled from obtaining his subsistence by manual labor by reason of his injuries above described, received in the service of the United States; and he therefore makes this declaration, for the purpose of being placed on the invalid pension roll of the United States. Goff aged 63 years, a resident of East Creek (Give P. O. address-street and number, if in city.), and _______ aged ___ years, a resident of _______(Give P. O. address-street and number, if in city.) in the County of _____ and State of ________, who, being duly sworn according to law, state that Charles P. Riel late Private in Co "F" 25" regiment of N.J. Vols, War of 1861, while in the line of his duty, at or near Battle of Fredericksburg in the State of Va. Did on or about the 21" day of December 1862, become disabled in the following manner, viz.: he contracted Kidney Trouble from the exposure and hardships of the service, which resulted in Gravel and symptoms observed are as follows; Said claiment was at diferent times while encamped at Falmouth Va, attended by the regamental Doctor and was not able to perform camp duty. (Here state the time and place and manner in which the wound or other injury was received. Describe the wound or injury, the part of the body wounded or injured, and all the circumstances attending it. If sickness, state time and place when contracted, what caused it, the name of the sickness, how it affected him, the symptoms then observed, &c.) That the facts stated are personally known to the affiant by reason of being present then and there, and that such personal knowledge was derived in the following way: from the fact that it was his [OVER.] Duty to detail guards and make daily repots of the condition of the company. The treatment received in the service was as follows: by the Regamental Doctor while encamped at Falmouth Va from Dec 20th 1862. (State all known about the soldier having received medical treatment for his disability while in service, giving time and place, if possible.) Affiant served as 1 Sergeant of Co. "F" of the twenty fifth (25) Regiment New Jersey Volunteers, from the first of January 1863, to the twentieth day of June 1863. And affiant is totally disinterested in this claim. Personally appeared before me, a Notary Public in and for the aforesaid County and State Morris Warwick and Seth L. Hand both of Dennisville Cape May Co N.J. (Names of Affiants, two or more) well known to me to be reputable and entitled to credit, and who, being duly sworn, declare, each in relation to aforesaid case, as follows: That affiants well know said soldier, and have known him for the past Thirty years and Thirty years years respectively: that soon after his discharge and return home, on or about the month of ___(First time seen.) 1863 they saw and conversed with him familiarly, and he was then suffering from Gravel and general debility and effects thereof, which, he then said, he had incurred in the army, in the line of duty; that his general appearance and symptoms then were as follows: such as are usually observed in such cases (Here state same, fully and precisely, how he looked, how affected, what symptoms he manifested, etc.) That he has continued to suffer from same, and the resulting effects thereof, with like symptoms, more or less, ever since, [OVER.] and by reason thereof, in each year, since discharged, has been incapacitated for manual labor to the extent of (1) one quarter of the time on an average, and is so now, to the extent of (1) one quarter, and that their means of knowledge as to foregoing facts are as follows: (2) that they have live neighbor to him a great part of the time and have seen and conversed with him a short intervals for 30 years that, in the opinion of the affiants, the same was caused by his said service in the army, and has not been aggravated nor prolonged by intemperance or other bad habits. Riel aged Fifty Five years, a resident of Dennisville in the County of Cape May and State of New Jersey well know to me to be reputable and entitled to credit, and who being duly sworn, declares in relation in his claim for pension as follows: My post-office address is Dennisville N.J. Dennisville Cape May County New Jersey (Give present address in full.) For five years immediately preceding my enlistment into the service of the United States on the day of Sept. 1862, I resided in the following named places: Dennisville N.J. (Give all the places in which you resided during the period above stated prior to your enlistment.) and my occupation was that of a Sawyer. Since my discharge from said service, I have resided in Dennisville NJ East Creek NJ Beeleplain(?) NJ Camdon NJ (Give the name of each place with date of any change of residence.) 1880, 1882, 1883, 1885, and my occupation has been that of a small farmer. From my said discharge to the present time, I have received the following medical treatment for disability alleged in my Declaration: I have been treated by Maurice Beesley M.D. George G Carl M.D. in 1863 & 64. Maurice Beesley & George G. Carl are both dead. Dr. Beesley died in 1881 or thereabouts. Dr. Carl died in 1890. (Give me the name and address of each physician employed, and the time when each commenced and ceased to treat you. If any of them are deceased, so state, and date of death, if remembered.) Since the origin of the disability for which pension is claimed, I have suffered with the following acute diseases: Gravel about twice month (Mention all attacks of acute disease, the time when such attacks occurred, their character and violence.) for which I was treated by Dr. L. J. Liggett of Goshen NJ June 1890 as he is treating me now. And during all the said time, my physical condition and ability to perform manual labor have been as follows: I was not able to work but have been compelled to work or starve. Can not give dates but have lost at least 1/4 of my time. (State whether you have performed any manual labor since your discharge, and if so, what kind, and whether at any time and for what period or periods, giving the dates as nearly as possible, you have been prevented from following your usual occupation.)
