Additional Comments:
The spouse's name is not listed as such.  There are two copies of this 
certificate.  On one, it shows Effie Moore as the informant and on the other 
it shows Ed? (difficult to read) Crawford.  Can't read the doctor's name.  His 
father's first name begins with an "H", but can't read the rest.
