In the Matter of the Registration of the Birth of Henry Louis Wilhelm

This cause came on to be heard upon the application of Henry Louis Wilhelm, 
praying that the court order the registration of the birth of said applicant, 
and the evidence in support thereof.  On consideration thereof, the court 
finds that notice of the hearing on said application has been dispensed with, 
and the court having personally examined the applicant in open court and 
having received clear and documentary evidence as follows:  Confirmation 
certificate signed by Christian Heddaens, pastor, showing date of birth of 
applicant; Honorable discharge from U. S. Army dated October 23, 1898, Spanish 
American War, showing applicant enlisted June 18, 1898; Western and Southern 
Life Insurance policy dated August 12, 1889 showing applicant's age as 13; 
Army account of pay and clothing dated September 1, 1898, shows applicant born 
in Ohio; Father's family Bible written in father's handwriting, shows name and 
date of birth od applicant; Marriage certificate, Probate Court, Vol. 29, page 
204, shows applicant to be 21 years old June 3, 1898.

It is therefore ordered that the birth of said applicant be registered in 
accordance with the above finding and that a summary, duly certified, of the 
above finding and order be forthwith transmitted to the State Director of 
Health at Columbus, Ohio, as provided by law
C. P. McClelland, Judge of the Probate Court of Franklin County, Ohio
