Claimant's post-office address: Cheshire, Ohio We hereby certify that in compliance with the requirements of the law we have carefully examined this applicant, who states that he is suffering from the following disability, incurred in the service, viz: Piles catarrh & deafness of left ear. He makes the following statement upon which he bases his claim for original [pension]: That at times he has Piles continually and that at times they course down. Also that he has pain in region of frontal sinus and discharge from the nose and that he is entirely deaf in the left ear. Upon examination we find the following objective conditions: Pulse rate: 72; Respiration: 18; Temperature: N [normal]; Height: 5 feet, 8 inches; Weight: 170 pounds; Age: 54 years. The rectum is congested and hemorrhoidal, vessels engorged with 2 _____ (illegible) Piles one half inch in diameter but not ulcerated. The anterior and posterior areas are congested of the throat. The membrane Tympani of Left ear is gone and he is totally deaf in that ear. Right normal in appearance and hearing normal in that ear. No other disability found. He is entitled to a 2/18 rating for the disability caused by piles, 4/18 for that caused by catarrh, and 10/18 for that caused by total deafness of left ear signed: James JOHNSTON, President W. On this first day of February, A.D., 1895, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, George H. BROWN aged 65 years, a resident of Cheshire in the County of Gallia and State of Ohio well known to me to be reputable and entitled to credit, who being duly sworn declared in relation tot he aforesaid case as follows: that he has known the claimant thirty five years and has lived within one half mile of him for the last ten years and have seen claimant on an average time a each week and know that he has suffered from catarrh of the head and throat, kidney trouble, rheumatism and complained of piles and deafness of the left ear and a general breaking down as to physical ability to do manual labor one third of the time if even that.
