The rest of the documents, except one letter as noted, were on preprinted forms - M.H.} April 17, 1888 State of Ohio, County of Gallia, SS: Before me the Clerk of the Common Pleas Court in and for aforesaid County personally came Charles MCMILLIN who being legally sworn says that his age is 38 years; that his Post Office address is Vinton Gallia County Ohio; that he is the identical man who under the name of Charles MCMILLIN enlisted in Co. D. 141 Ohio National Guards as a private, on the 2d day of May 1864 to serve one hundred days under the following discription age 18 years, 5 ft 2 inches high, sandy hair, hazle eyes, & light complexion and a farmer; that he was honorably discharged from said service on the 3d day of Sept 1864 by reason of expiration of term of service; that while a member of said Co. in the U.S. service and in the line of his duty at Mud Bridge W.Va. on or about the 6th day of July A.D. 1864 he was disabled by a wound in the left side of the neck by the accidental discharge of a revolver in the hands of a comrade; that he was treated at the time by the Regtl. Surgeon James JOHNSTON and after his discharge by Dr. JOHNSTON his Regimental surgeon for heart disease also which was a result of said wound; that the ball from said revolver entered his body and has never been removed; that he is materially disabled for the performance of manual labor by said wound and the results thereof and makes this declaration for the purpose of having his name placed on the pension rolls of the United States; that he has not been employed or paid in the military or naval service of the US since the 3d day of Sept 1864; that since his discharge he resided several years near Cheshire Gallia Co O and since that at Vinton Gallia County Ohio; and that he hereby appoints Hiram WILLCOX of Vinton Gallia County Ohio his attorney to prosecute his claim. signed by: Charles MCMILLIN H.R. Sir: I have the honor to request that you will furnish from the records of the War Department a full Report as to the service, disability, and hospital treatment of Charles MCMILLIN, who, it is claimed, enlisted May 2nd, 1864 and served as private in Co. D, 141 Reg't Ohio Inf, and was discharged at placement stated Sept. 3rd, 1864. While serving in Co D, 141 Reg't Ohio Inf, he was disabled by gunshot wound in the left-side of neck inflicted by a revolver in the hands of a comrade, received July 6th, 1864. Contracted heart disease at same time and place and was treated in hospitals of which the names, location, and dates of treatment are as follows: Regimental Surgeon. Very respectfully, John C. Charles MCMILLIN, a Private of Company D, 141 Regiment Ohio Volunteers, was enrolled on the 2 day of May, 1864, at Cheshire, Ohio for 100 days and is reported: on roll May & June /64 present. Muster out roll of company dated Sept 3/64 Gallipolis Ohio shows him mustered out with company at said date & place a private, Regtl. Returns July /64 not reported about sick or otherwise. Regtl. Col., 141st OVI, November 30, 1888 State of Ohio, County of Franklin, SS: In the matter of the application for pension of Charles MCMILLIN late of Co D. 141st Ohio Vols On this 30 day of November A.D. 1888; personally appeared before me, a Notary Public in and for the aforesaid County, F.W. HAMPTON aged 62 years, whose Post Office Address is Columbus Ohio, well known to me to be respectable and entitled to credit, and who, being legally sworn, declares in relation to the aforesaid case as follows: That he has been well and personally acquainted with Claimant for about 26 years, and that on or about the 6th day of July AD 1864 I was Lieutenant Colonel of the 141 O. National Guards and was at the time stationed at Mud Bridge West Virginia and the said Charles MCMILLIN was then a private in Captain A.O. MAUCK's Company (not now ___ erubancy His ____ of the Company) and said Charles MCMILLIN then being on duty and I struck ??? acting as orderly for me, was accidentally shot be a comrade by the name of Frank SOUVERICH(?) in the back side of his face or neck and at the time gave me great alarm as his blood flowed profusely, and the ball did not go through or come out but disabled the said Charles MCMILLIN to such an extent that he did not so much of any further duty. Tells often he was mustered out of the service which occurred in September about the 4 or 5 of the same year. As to the extent of the injury I only know by seeing him frequently since and hearing him complain and apparently suffering. Claimant was strictly in the line of his duty when he was wounded as aforesaid. (stamped, not signed) F.W. HAMPTON PAGE========== PENSIONER DROPPED U.S. PENSION AGENCY, Columbus, Ohio 8 Oct., 1895 I hereby report that the name of Charles MCMILLIN Pvt D - 141" O, who was a pensioner on the rolls of this Agency, under Certificate No. 453154, and who was last paid at $8, to 4 Mch, 1895, has been dropped because of information from PM {Post Master} Vinton O that pensioner died 26 Mch 95. PAGE========== DECLARATION FOR WIDOW'S PENSION State of Ohio, County of Gallia: On this 13th day of April, A.D. one thousand eight hundred and ninety-five personally appeared before me, a Notary Public within and for the County and State aforesaid, Elizabeth MCMILLIN, aged 36 years, a resident of the Village of Vinton, County of Gallia, State of Ohio, who being duly sworn according to law, declares that she is the widow of Charles MCMILLIN who enlisted under the name of Charles MCMILLIN at Gallipolis Ohio, on the 2d day of May A.D. 1864, in Co. D, 141st Regiment Ohio Volunteers and served at least ninety days in the late war of the Rebellion, who was honorably discharged Sept 3rd 1864, and died on the 26th day of March 1985. That she was married under the name of Elizabeth KOONTZ, to said Charles MCMILLIN, on the 10th day of February 1878, by H. P. HANNAH, J. P., at Raccoon Township Gallia Co, Ohio. That she has not remarried since the death of the said Charles MCMILLIN. That she is without other means of support than her daily labor. That names and dates of birth of all the children now living under sixteen years of age of the soldier are as follows: Forrest A. MCMILLIN, born January 31st, 1882 George W. McMILLIN, born March 24th, 1884 Jennie B. MCMILLIN, born August 25th, 1889 That she has heretofore applied for pension and the number of her former application is..... No prior application has been filed. That she makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of June 27, 1890. DUSTIN, M.D., April 27, 1895 State of Ohio, County of Gallia, SS: In the Pension Claim of Elizabeth MCMILLIN widow of Charles MCMILLIN late of Co D. 141st Ohio Vols Personally appeared before me, a Notary Public in and for the aforesaid County, J.B. DUSTIN a citizen of Vinton Gallia Co Ohio whose Post Office Address is Vinton Gallia County Ohio, well known to me to be respectable and entitled to credit, and who, being legally sworn, declares in relation to the aforesaid case as follows: That he is a Physician, and that he has been acquainted with said soldier for about 15 years, and that this is to certify that he delivered Mrs. MCMILLIN of her last child on Aug. 25, 1889. She being the lawful wife of Charles MCMILLIN deseased and that the above statement is in my own hand written. PAGE========== GENERAL AFFIDAVIT of Armenia MCMILLIN, June 25th, 1895 State of New York, County of Erie, SS: In the Matter of claim for widow's pension of Elizabeth MCMILLIN widow of Charles MCMILLIN late private of Co D. 141st Ohio Vols On this 25th day of June, A.D., 1895, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Mrs. Armenia MCMILLIN aged 38 years, a resident of Buffalo in the County of Erie and State of New York well known to me to be respectable and entitled to credit, and who, being legally sworn, declares in relation to the aforesaid case as follows: I hereby certify that said George and Forest MCMILLIN are the legitimate children of Charles and Elizabeth MCMILLIN that said George and Forest MCMILLIN were born at Bairds Furnace Perry County Ohio. That Forest MCMILLIN was born at above named place January 31st 1882 and that said George MCMILLIN was born at said place March 24, 1884 and that I was present at time of their births. PAGE========== GENERAL AFFIDAVIT of John MATTHEWS, October 11, 1895 State of Ohio, County of Gallia, SS: In the Matter of claim for widow's pension No. 616,137 of Elizabeth MCMILLIN widow as of Charles MCMILLIN Co D. 141st Ohio Vols On this 11th day of October, A.D., 1895, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, John T. MATTHEWS aged 42 years, a resident of Vinton in the County of Gallia and State of Ohio well known to me to be respectable and entitled to credit, and who, being legally sworn, declares in relation to the aforesaid case as follows: That he has been well acquainted with the above named claimant from her childhood to the present, that her maiden name was Elizabeth KOONTZ that she was never married prior to her marriage with Charles MCMILLIN her late husband and that she has not been married since the death of said Charles MCMILLIN. Affiant further states he will knew her late husband Charles MCMILLIN during his lifetime that said soldier was never married prior to his marriage with the above named claimant and that they always resided together as man and wife from date of marriage until date of said soldiers death. Affiant further states that all the children of said soldier and claimant are living. Affiant states that all property real personal or otherwise owned or provised by the claimant at present consists of a house and small town lot situated in the village of Vinton Gallia County Ohio upon which claimant resides the actual cash value of which is about $250.00 dollars. That she derives no income from said property or from any other source and that no person is uder any legal obligation to contribute to her support. That affiants means of surviving the above facts are derived from being a close neighbor and residing in same Village with claimant. Affiant further states that the above statement was foresaid in his presence and from his own dictation by Robt B. CARTER at Vinton Ohio on the 11th day of Oct 1895. That in making the same he was not aided or prompted by any written or printed statements but from his own knowledge of the facts contained herein. His Post Office address is Vinton Gallia County Ohio. He further declared that he has no interest in said case and is not concerned in its prosecution. Signed by: John T. MATTHEWS PAGE========== GENERAL AFFIDAVIT of Elizabeth MCMILLIN, October 15, 1895 State of Ohio, County of Gallia, SS: In the Matter of widows claim for pension No. 616,137 of Elizabeth MCMILLIN widow as of Charles MCMILLIN Co D. 141st Ohio Vols On this 15th day of October, A.D., 1895, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Elizabeth MCMILLIN aged 36 years, a resident of Vinton in the County of Gallia and State of Ohio well known to me to be respectable and entitled to credit, and who, being legally sworn, declares in relation to the aforesaid case as follows: That she is the claimant in the above cited case, and states as follows that Forrest MCMILLIN and George C. MCMILLIN, born respectively Jan 31st 1882, and March 24th 1884 at Bairds Furnace Perry County Ohio, are the legitimate children of claimant and her late husband Charles MCMILLIN. That the attendant physician who was present at the births of said children, was Dr. Gua(?), who is dead; that the only other person now living who was present during her confinement is Mrs. Arminia MCMILLIN, whose Post Office is Buffalo Erie County State of New York, that no record of the births of said children appear upon the records of Perry County Ohio, claimant further states that her property on real estate containing 6-1/2 acres as heretofore described by copy of Auditor of Gallia County Ohio and filed with her declaration June 17th 1895 has been sold and that she holds no primary interest in the same. That the above statement was performed in presence of the claimant and from her own declaration that she was not aided or prompted by any statement prepared or dictated by any other person but from her own person knowledge of the facts as above stated.
