The rest of the documents, two letters as noted, were on preprinted forms - M.H.} April 29, 1889 State of Ohio, County of Gallia, SS: Before me the Clerk of the Common Pleas Court in and for aforesaid County personally came Samuel R. MCMILLIN who being legally sworn says that his age is 58 years; that his Post Office address is Vinton Gallia County Ohio; that he on the 20 day of February 1865 enlisted under his aforesaid proper name in Co. G. 195 Ohio Inft. Vols as a private to serve one year and was honorably discharged from said service on the 18th day of December 1865 being at the time of enlistment 34 years old. 5 ft 8-1/2 inches high and a farmer, and having dark hair, dark eyes and dark complexion; that he was never employed or paid in the military or naval service of the U.S. except from Feb 20 to December 18th 1865 as aforesaid that while in said service and in the line of duty at Charlestown Va about the 1st of April 1865 he was disabled by diarrhea and piles which have continued and now disable him to a very material extent for the performance of manual labor and he therefore makes this his first application for pension; that his only hospital treatment was in field hospital for one day. That his residence before and since his aforesaid service was and has been near Cheshire & Vinton in Gallia County Ohio; except from 1870 to 1876 he resided at Keystone Jackson County Ohio during which years he worked at the Iron Furnace and that his occupation except from 1870 to 1876 has been farming; that he hereby appoints Hiram WILLCOX of Vinton Gallia County Ohio his attorney to prosecute his claim. He was mustered out a pvt with Co. at Alexandria Va Dec. 18, 1865. Return for March '65 does not report him absent. No further information. MCMILLIN who was ENROLLED on the 20th day of February, 1865 in Company G of the 195th Regiment Ohio Volunteer Infantry as a private in the war of the rebellion, and served at least ninety days, and was HONORABLY DISCHARGED at Alexandria Va, on the 18th day of December, 1865. That he is Totally unable to earn a support by reason of chronic diarrhea Rheumatism and Piles. MCMILLIN to be the identical person of that name who enlisted or volunteered as a private in Company G 195th Regiment Ohio Infantry vols., and who was discharged at Alexandria Va, on or about the eighteenth day of December, 1865 by reason of the muster out of aforesaid Company. That the said Samuel R. MCMILLIN while in the line of his duty, at or near Charlestown in the State of Virginia did, on or about the Spring the year 1865, become disabled in the following manner, viz: By rheumatism and piles and there after at intervals during the term of his service he complained of rheumatism and piles and seemed to be to some extent disabled thereby and was for that reason at times excused from duty and was also granted a sick furlough. That the facts stated are personally known to the affiant by reason of his being 1st Lieut of said Co. G. and from being present with the company when the herein before stated facts transpired. And deponent further state that he was well acquainted with the claimant, having known him for at least during said service and further, that his knowledge of the facts above stated are derived from said acquaintance, and from having served as 1st Lt. of Company G of the 195th Regiment of Ohio Inft volunteers from the 18th day of March 1865 to the 18th day of December 1865. And deponent further state that the claimant was a sound and able-bodied man at and prior to enlistment, so far as he knew, and that he is totally disinterested in this claim. MCMILLIN late of Co G. 195th OVI Personally appeared before me, a Notary Public in and for the aforesaid County, Wm SYLVESTER a citizen of Wellston O whose Post Office Address is Wellston Jackson County Ohio, well known to me to be respectable and entitled to credit, and who, being legally sworn, declares in relation to the aforesaid case as follows: That he is a Physician, and that he has been acquainted with said soldier for about 20 years, and that I treated said soldier. Prescribed for him a few times for diarrhea I think it was, between the years 1871 and 1874. I have no record of treatment as he paid me at the times. MCMILLIN, Priv Company G, Reg't: 19 O Vol Post Office address: Vinton Ohio Date of examination: July 17, 1889 We hereby certify that in compliance with the requirements of the law we have carefully examined this applicant, who states that he is suffering from the following disability, incurred in the service, viz: Diarrhea & Piles Pulse rate per minute, 78; respiration 18; temperature, none; height, 5 feet 9-1/2 inches; weight 135 pounds; age 59 years. He makes the following statement upon which he bases his claim for: Original. That he has frequent attacks of diarrhea and that between attacks he is constipated. Upon examination we find the following objective conditions: The tongue is coated fissured and papilla raised with red edges to tip. Abdomen flat and tender with tenderness over the stomach and liver but no enlargement of the latter no tenderness or enlargement of the spleen Rectum congested with hemorrhoidal vessels engorged with several intestinal Piles and once external as large as a hazlenut he is frail in flesh and anemic. He is, in our opinion, entitled to a 6/18 rating for the disability caused by chronic diarrhea and resulting piles. He further declares that he has been a practitioner of medicine for 34 years, and that he has no interest, either direct or indirect, in the prosecution of this claim. Signed: Wm SYLVESTER. MCMILLIN for about 30 years, and that I have lived within one fourth of a mile of him for the last sixteen years and have saw him almost Daily ever since and know from Personal knowledge and intimate aquaintence that he is Disabled to a great extent he has always been complaining ever since the War With Diareah Rheumatism Pain I his Back and Piles. I have saw him sometimes when he could not sit down and I have saw him when he could not get up on acount of his Back and from Personal knowledge and intimate aquaintence I know Samuel R. MCMILLIN On this 1 day of April, A.D., 1891, personally appeared before me, a Notary Public in and for the aforesaid County, George W. JACKSON aged 51 years, whose Post Office address is Vinton Gallia County Ohio, well known to me to be respectable and entitled to credit, and who, being legally sworn, declares in relation to the aforesaid case as follows: That he has been well and personally acquainted with Samuel R. MCMILLIN for about 30 years, and that I have been intimately aquainted for about sixteen years lived within one half mile of him for that length of time and he has always been complaining since that time with Rheumatism Piles and Diarrhea and I had saw him frequently During the time I have known him that he was unable to do anything on acount of said Disabilitys. I saw him at Different times he was confined to his bed. I cannot give exact dates of the times but was during the Last sixteen years Whilst We lived neighbors. I have all Reason and do Believe from Personal knowledge and intimate aquaintence With Samuel R. MCMILLIN that he has been for the last sixteen years a Really aflicted man and is still suffering with the above Disabilitys. Signed: George W. F. SOLES aged 44 years, a resident of Ewington in the County of Gallia and State of Ohio, who being duly sworn according to law, state that he has been acquainted with Samuel R. MCMILLIN applicant for Invalid Pension, and know the said Samuel R. MCMILLIN to be the identical person of that name who enlisted or volunteered as a private in Company G 195th Regiment of Ohio vols., and who was discharged at Alexandria Va on or about the 21st day of December, 1865 by reason of honorable discharge of the Regiment. That the said Samuel R. MCMILLIN while in the line of his duty, at or near the Shenandoah in the State of Virginia did, on or about the last days of May, 1865, become disabled in the following manner, viz: by diarrhoea and was furloughed and was at home about four weeks and after the return of said Samuel R. MCMILLIN from his home and furlough he was excused from duty at least one third of the time on account of disability from chronic diarrhoea and piles with which he was a continuous sufferer after his aforesaid return from home. That the facts stated are personally known to the affiant by reason of being a member of Co. G. 195. O.V.I. aforesaid and being present with the Company in May 1865 and until the Regt was discharged in December 1865. That since discharge they have been intimately acquainted and have seen each other frequently and several years they resided near each other; that said Samuel R. MCMILLIN has complained and has been disabled by chronic diarrhoea and piles every year since discharge and some years affiants knowledge of the case has been many times a year. He makes the following statement upon which he bases his claim for original That he has diarrhoea about one third of the time. discharges slimy and at times streaked with blood. during intervil bowels constipated and bloat. and has piles which at times protrude and bleed. and has rheumatism in small of back and hip. joints do not swell but are stiff and sore. Upon examination, we find the following objective conditions: Pulse rate, 70; respiration, 18; temperature, 98-1/2; height 5 feet 10-1/2 inches; weight, 130 pounds; age, 62 years. General health and nutrition poor. Finger coated with brownish p_____ and edges red. conjunctiva yellowish, skin sallow. Tenderness over stomach and along neck of colon. Tympanitis over stomach and along neck of colon. Liver tender & slightly enlarged. Spleen normal. Dis. from chr. diarrhoea. Four eighteenths Rectum inflamed hemorrhoidal vescles protruding external. Oval shape and about 1/4 inch in diameter. Turgid not bleeding. digital examination shows few internal piles about size of peas. turgid not bleeding. Dis from piles Four eighteenths. Shoulder - elbow - wrist - knee & ankle joints normal. Tenderness over spine in lumbar region. Spinal muscles contracted & tender. tenderness extending down with hips along back of sciatic nerve. Claimant has lumbago - he is Four eighteenths. Heart sounds feeble otherwise normal. ______ within normal limits. Kidneys and urinary organs normal. Except as above described no disability of any kind found. He is, in our opinion, entitled to a 4/18 rating for the disability caused by chronic Diarrhoea, 4/18 for that caused by Piles, and 4/18 for that caused by Lumbago. Signed: Geo. BEAN, President J. H. G 195 O.V.I. and Have lived within 2 miles of Him for 14 or 15 years and Saw Him frequentley during the rest of the time Since our discharge and from His looks and Complaining He is not an able bodied man. But as to the extent of His disability I am not able to Say neither can I discribe the simtoms or its results.
