Elliott, who, it is claimed, enllisted October 7th, 1862, and served as private in co. D 76 Reg’t OVI and was discharged at Black River Miss Aug 4, 1863 he was disabled by diarrhea, disease of heart stomach liver lungs and debility, results of Malaria. Elliott private Co D 76th Ohio vols Personally came before me , a Notary Public in and for aforesaid County and State, ________residence Utica, Licking County post office ________persons of lawful age , who, being duly sworn.......I have to say that I have known him since his childhood and that prior to his Service in the army during the late war he was a healthy robust young man after his return form the army in the latter part of the Summer of 1863 he appeared to me for medical treatment. I found him suffering from Chronic malarial poisoning, having fever and chills and great derangement of the Liver being considerably jaundiced to the best of my recollection he was also troubled at teh same time with chronic diarrhea and that both these troubles recurred occasionaly for Several years. I am not related to claimant and an mot interested in the prosecution of this claim. that I make this affidavit as a copy of and to take the place of one made and executed by me December 7th 188 in the above titled claim. and further that I have no interest in said case. Elliott private Co D 76th reg’t Ohio Vols., personally came before me, a Notary Public etc etc.that his age is 50 years; that he is the identical person who served as a 1st Lieut (??) in Co D 76th Reg’t Ohio Vols., that on or about the ____ day of , 186____while in the line of duty and without fault or improper conduct on his part, aat or near Black River, State of Mississippi said soldier incurred I remember very Distinctly that the aplicant was Sick with what was known as Diarhea and was on the sick list Sometimes before h was discharged in August 1863 Our camp at that time was very Sickly Black River being a very marshy place and not aat all healthy. I was in the Commissary Department at that time but a number of the Same company and well remember the aplicant who was a good Soldier when able for duty and I well remember when he was discharged on the 4th of August 1863 at Black River Miss. Elliott private Co D 76th Ohio Vol Personally came befdore me, a Notary Public etc, R. B. Corbett M. D. age 63 year residence Licking Co Ohio Post Office St. Louisville, Ohio...declare..as follows: I have been in the regular practice of medicine for 40 years and have been acquainted with Hugh M. Elliott 3 years before his going into the army during the late war of the rebellion. Said Soldier before enlistment was a Sound man performing all kinds of labor on a farm and if Said Soldier had not been Sound I most assuredly would have known about it, but after his return home August 4, 1863 I was called within 48 hours after arrival home Said Soldier was down with malarial fever, Chills, derangement of the liver jaundice and also Chronic diarrhea, with his complication of diseases Said Soldier had run down with general disability in Several weeks he recruited some in health so as to be out Some. I treated the best I could under the circumstances but later in the fall he employed Dr Dicus to attend him he being Several miles from me. I soon lost sight of him as his Doctor. I have see Said Soldier Several times at various periods of time since he has been treated by others at each and every time that I have seen him he complained of Stomach and Heart trouble and Chronic diarrhea. Since I treated him in 1868 I know but little about Said case only by what he has told me. I am not related to claimant and not interested in the prosecution of this claim. that on December 14th 1888 I executed an affidavit in this claim similar to the foregoing which if lost I desire to replace with this Statement R. Elliott private Co D 76th Ohio vols personally came before me, a Notary Public in, and for aforesaid County and State, the claimant residing in Licking Co, Ohio, Post Office Purity, Licking County, Ohio..being duly sworn, declare...as follows: Says that during Service he was treated during Service by Assistant Surgeon Several .....for chronic diarrhea, chills and fever and which Assistant Surgeon of the 76th Ohio vols is now dead for that reason claimant is unable to furnish medical evidence to Show the incurrence of the Said disabilities in the Service, but that he has furnished to the pension department the affidavits of Commissioned Officer and comrades to Show origin of above named disabilities in Service and in the line of duty and asks that Said affidavits be taken in lieu of medical evidence to establish his claim for pension, that he has furnished the affidavits of Dr R. P. Corbett of St. Louisville, Ohio Executed December 14, 188, Dr H. I. Dicus of Utica, Ohio Executed December 7, 1888, Dr T Arthur Wright of Americus, Kansas. the date of the Execution of which claimant has forgotten. Dr J. F. Shroutz of Martinsburg, Ohio, Executed February 17, 1889 all of which medical evidence goes to show treatment for the above named disabilities from Service to the time of the execution thereof. D, 76 Regt Ohio Vol Infantry and was honorably discharged Aug 4, 1863, having served ninety days or more, or was discharged for, or died in service of the United States of a disability incurred in the service in the line of duty, during the CIVIL WAR, and who DIED Mar 27, 1923, at Newark, Ohio......
