   APPLICATION FOR LETTERS OF ADMINISTRATION

The State Of Ohio, Lorain County           PROBATE COURT
  Catherine Mezlak, being duly sworn says that Rudolph Mezlak, aka late a 
resident of the City of Lorain in said County, died on or about the 7th day of 
May 1950, at Cleveland, Ohio leaving no surviving spouse, of the age of years, 
whose place of residence and P.O. Address is______ and the following persons 
his known next of kin:

  The undersigned asks to be appointed Administratrix of the Estate of said 
decendent and on her oath aforesaid says, said estate consists of :
Personal Property of the probale value______________________$2000.00
Annual Real Estate Rentals of the probale value of__________$
And Real Estate of the probable value of____________________$
                            Total,__________________________$2000.00
The following is a statement of all indebtedness of the deceased had against 
the undersigned:
_______________________________Amounting to $___________, included in the 
amount of personal property above stated.
  There is not to her knowledge any Last Will and Testament of such intestate. 
The undersigned offers a bond as such Administratrix in the sum of $4000. with 
New Amsterdam Casualty Co.____________ and as sureties thereon.

                      WAIVER
  We, the undersigned, surviving spouse and the next of kin of said Rudolph 
Mezlak, aka late of said county, deceased, who are resident of said county, 
hereby voluntarily renounce the administration of said Estate and recommend 
the appointment  of Catherine Mezlak as Administratrix.
