I, 149 Regiment, Ohio Vols, a resident of the P.O. of Rainsboro County of Highland State of Ohio who, being duly sworn according to law, declares that he is a pensioner of the United States under the Act of June 27, 1890, enrolled at the Columbus, Ohio Pension Agency, at the rate of Eight dollars per month, by reason of partial inability to earn a support by manual labor, his Pension Certificate being numbered 725886. That he believes himself to be entitled to an increase of pension on account of the Disabilities Heretofore Alleged, namely: Catarrh and Rheumatism and that said disabilities have increased in severity and resulted in disease of heart and nervous prostration. Also on account of disease of kidneys and general disability incurred (blank). That said disabilities are not due to vicious habits, and are to the best of his knowledge and belief permanent in character and disable him from earning a support by manual labor.
