On this 31 day of May, A.D. 1904, personally appeared before me Robert Clarke, aged 72 years, a resident of Rainsboro, County of Highland, State of Ohio, who being duly sworn, declares that he is the identical person who was enrolled at Chillicothe Ohio under the name of Robert Clarke on the 2 day of Mar, 1864, as a Corporal in Co. I, 149 O.V.I. in the service of the United States, in the war of the rebellion, and served at least ninety days, and was HONORABLY DISCHARGED at Camp Dennison Ohio, on the 30 day of August 1864. That he is in part incapacitated for earning a support by manual labor by reason of age. That he was not employed in the military or naval service prior to May 2nd, 1864. That he has not been employed in the military or naval service since Aug. 30th, 1864. That he is a pensioner. That he has heretofore applied for pension By lett No. 725886. Philip A. Roads Company I 149 Regiment of Ohio National Guards Volunteers, who was enrolled on the second day of May 1864 in the service of the United States, in the Civil war, and was Honorably Discharged at Camp Denison, on the 30 day of August, 1864. That he also served (blank). That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 5 feet 8 inches; complexion, dark; color of eyes, blue; color of hair, dark; that his occupation was Farmer; that he was born October 28, 1832 at Chillicothe, Ohio.
