State of Ohio County of Portage On this 2nd day of April , A.D. , one thousand eight hundred and seventy 8, personally appeared before me, Clerk of the Com. Pleas, a court of record within and for the county and State of aforsaid, Conrad Mack aged 54 years, a residend to Charlestown, county of Portage, State of Ohio , who , being duly sworn according to law , declares that he is the identical Conrad Mack who was ENROLLED on teh 20 day of August , 1862, in company i of the 115 regiment of Ohio Inf. Vols. commaned by Capt. (C or O?) Buckingham, and was honorably DISCHARGED at Mumfreesboro Tenn. on the 22 day of June, 1865; that his personal description is as follows: Age, (54 or 57 ? ) ; height, 5 feet 3 inches; complexion, light; hair, light ; eyes gray. That while a member of the organization aforsaid , in the service in the line of his duty at Mumfreesboro , in the State of Tenn. on or about the 1st day of March , 1865, he caught a heavy cold by exposure while on duty which settled in the kidneys and back causing disease of the spine and kindneys for which he was discharged from service. That he was treated in hospitals as follows: Mumfreesboro Tenn in Bria___? April 1865 That he has not been employed in the miliatary or naval service otherwisee than as stated above That since leaving the service this applicant has resided in the ____ of Charlestown & Randolph in teh State of Ohio, and his occupation has been that of a (none). That prior to his entry into the service above named he was a man of good, sound, physical health being when enrolled a Tailor . That he is now totally disabbled from obtaining his substence by mannual labor, by reason of his injuries, above described, received in the service of the United States; and he therefore makes his declaration for the purpose of being placed on teh invalid pension roll of the United States. He herby appoints, with full power of substitution and revocation, ? A. Dimmick of Washington D.C., State of _____ , his true and lawful attorney to prosecute his claim. Snyder ********************** ************** War Department 253560 ADJUTANT GENERAL'S OFFICE Washington, D.C. , July 5, 1878 Sir: I have the honor to acknowledge the receipt of your letter of the 24 day of June, 1878, requestion a "Statement of Service" of Conrad Mack The following information has been obtained from the files of this Office, and is respectfully furnished in reply to your inquiry : It appears from the Rolls on file in this Office, that Conrad Mack was enrolled on the 17 day of Aug. , 1862, at Randolph, in Co. i , 115 Regiment of Ohio Volunteers, to serve 3 years, or during the war. On the Muster Roll of Co. i of that Regiment for the months of Sept. and Oct. , 1862, he is reported present, and so reported to June 30,' 63, July and Aug. '63 absent on furlough since Aug. 17, ' 63 Sept and Oct. ' 63, present and so reported to April 30, '65. Return for March ' 6 does not report him absent. Regt. was at Murfreesboro Tenn. , March 1865. Of Pensions Washington D.C. ********************* **************** GENERAL AFFIDAVIT State of Ohio , County of Portage In the matter of Conrad Mack's Pension application on this 13th day of May A.D. 1884; personally appeared before me a Notary Public in and for the aforsaid County, duly authorized to administer oaths , Jacob Nise, aged 49 years, a resident of Randolph in the County of Portage and State of Ohio whose Post Office address is Randolph Portage County Ohio and _____, aged______years, a resident of ___ in the county of ______ and State of _______ whose Post Office address is ____ well know n to me to be reputable and entitled to credit, and who , being duly sworn, declare in relation to aforsaid case as follows: I have known Conrad Mack for over 23 years, having been a neighbor before the war and since the war until he moved to Charlestown Portage County Ohio. That before the war he was an able bodied man. That he was healthy and labors as men in good health do. ???? in 1860 he ???? some tim ein Aug. , and was sick , complaning of his back, that soon after his return , can't remember just the day, that ???? Milton Shouk who is (west?) somewhere now , sat up with him for three or four nights changing off with others. That he was entirely helpless, complaining in his back and head. He was then ???? ??? That before the war he labored for different farmers of which affiant knows and where affiant saw him that he did a good days work. That since the war and since his return from the army, he has not been able to do anything. That he saw said Mack at different times attempt to labor at light work since war, such as ?? corn on plant?? corn ??? but could not stand it . Affiant says he was very familiar with said Mack and knows that when Mack came home from the Army , he was sick, because he had every appearance of a sick man and he complained in his back and lungs. Affiant further declairs that he no interest in said case, and he is not concerned in its prosecution. Jacob Nise *************************** ********** Pensioner Dropped U. S. Pension Agency Columbus, Oh. Feb. 21, 1890 Hon. Greer B. Raum Commissioner of Pensions Sir: I hereby report that the name of Conrad Mack , who was a pensioner on the rolls of this Agency under Certificate No. 361,996 and who was last paid at $16, to 4 Dec., 1889, has been dropped because of information from S. C. Mack, Charlestown, O. , that pensioner died 19, Jan. 1890 Very respectfully , ????? can't read, pension agent **************************** ************** GENERAL AFFIDAVIT On this fourth day of June? , A.D., 1891, personally appeared before me, a Justice of Peace in and for the aforesaid Cunty, duly authorized to administer oaths Thomas (?B or R.) King and wife, aged 74 and 72 years whose Post Office address is Charlestown, County of Portage and state of Ohio, well known to be reputable and entitled to credit, and who, being duly sworn declares in relation to aforsaid case as follows: We have known & been well aquainted with Mary MACK for 16 yrs. We live within 20 rods of where she resides and neighbors with her. During the past years we have frequiently called at her home, as she is sick a good deal. We see her an average of 7 times a week. We know from our knowledge and observation that Mary Mack has no means of support. That she is not able to earn a living by work, does no t go out of the home much & is confined to her bed a good deal of the time of late and under the care of the doctor. A grandson lives with her and helps with housework. Thomas MACK, her son , furnishes her with food , clothing , fuel , light etc. He does this because she is his mother and there is no one else to support her, it is all the support she has , he also pays the doctor's bills. All of which amount to $2.50 or $3.50 per week. She has no income whatever. We have no interest in the prosecution of this claim. We further declair that we have no interest in said case and are no t concerned in its prosecution, Thomas (? B. or R.) King Sally M. I knew said Mary Mack at the time of the death of her husband and have known her ever since. I have lived in the same house with her for ninteen years and have seen her every day. I see from my associations with her & know that she has not remarried since the death of her husband Conrad Mack. He further declares that he has no interest in said case and is not concerned in its prosecution. Clyd D. Freeman, aged 51 years, whose post office address is Ravenna, County of Portage, and State of Ohio, well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to aforsaid case as follows: There is no property assesed against Mary Mack in Charlestown Twp., Couny of Portage and State of Ohio for the year 1891 as shown by the tax duplicater or records of said County of Portage. I further declare I have no interest in said case and am not concerned in its prosecution S.R. Hatfield aged 49 years, whose Post Office is Charlestown, County of Portage, State of Ohio. I have known Mary Mack for nine years. I knew her at the time of her husbands death. I have lived near her for nine years and have seen her ____ ___ days, and know from my associations with her, that she has not married since the death of her husband. She further declares that she has no interest in said case and is not concerned in its prosecution. Mary P.
