He had been attending to his work as usual the day of his death. He had eaten his supper and retired but feeling the usual symptoms of an indigestion had arisen and came down stairs. REED NEIGHBOR AFFIDAVIT On this 17th day of April, A.D. 1905, personally appeared before me Henry YANT aged 64 years a resident of Canton in the County of Stark and State of Ohio, well known to me to be reputable and entitle to credit, and who being duly sworn, declares in relation to aforesaid case, as follows: That he was not related to the Claimant or the Soldier, that he first became acquainted with Samuel J. REED about Aug. 1862 when we both enlisted in Co. B . 115 Regt. O.V.I. and served there about three years. He was then about 18 years old. During the past fourteen years we have lived close neighbors in same precinct, Second Ward, City of Canton, Ohio, and affiant knows that Samuel J. REED was only married the one time. Was not acquainted with Margaret A. REED prior to her marriage to Samuel J. REED. Samuel J. REED died on Saturday, April 8th, 1905 at his residence, 641 North Cherry Street, Canton, Ohio, leaving a widow and six children. During our residence as neighbors in Canton, Ohio we met frequently and in conversations he often complained to me of chronic diarrhea with bloating of stomach and choaking in his chest. REED the ex soldier and was born May 11th 1838 and brother Samuel was the third child younger and was born May 13th 1844. Our residence was in Bethlehem Township, Stark Co., Ohio. We had come to Canton, Ohio before the war of Rebellion. I knew that Brother Samuel J. REED was married about 30 years ago to Margaret A. BACHTEL and that this was his first and only marriage. Was not acquainted with Margaret A. BACHTEL before her marriage to my brother. They have resided in Canton, Ohio, and have a family of six children and they were never separated or divorced. My Brother Samuel J. REED died this month in Canton, Ohio but I was unable to attend his funeral on account of my Infirmities and age. Never heard of any separation or divorce between them and know that they have a family of children but am not so well acquainted with them. O.V.I. and honorably discharged August 22, 1865 having served ninety days or more during the late war of the rebellion. REED who was enrolled under the name of Samuel J. REED at Canton, Ohio on the 11th day of August, 1862 as a private in Company "B" 115th Regt. O.V.I. and honorably discharged August 22, 1865 having served ninety days or more during the late war of the rebellion. That the said soldier died April 8, 1905, at Canton, Ohio: that she was not divorced from him, that she has not remarried since his death; and that she is without other means of support than her daily labor and an actual net income not exceeding two hundred and fifty dollars per year. GENERAL AFFIDAVIT State of Ohio, County of Stark In the matter of Original Invalid Claim No. 981634 Samuel J. REED, Company B 115th Regiment V.O.I. On this 6th day of February, A.D. 1891, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Alonzo S. SLUSSER aged 44 years, a resident of Osnaburg in the County of Stark and State of Ohio, well known to me to be reputable and entitled to credit, and who being duly sworn, declares, in relation to aforesaid case, as follows: That he became acquainted with the claimant Samuel J. REED as a member of Co. B 115 Regt. Ohio Inf. Vols, was with him at Block House No. 6 in the summer of 1864 and remember well that we nearly all suffered from camp diarrhea there which we attributed to having to drink the creek water and that Samuel J. REED was one that had it pretty bad. I was not captured with them the last of Aug 1864 as I was detailed the day before to go back to Head Quarters for the mail. After this I served at Normanda Station Tenn and did not see much of the claimant there until about time of our discharge from service in June 1865. He did not look stout and hearty then. SLUSSER GENERAL AFFIDAVIT State of Ohio, County of Stark In the matter of Original Invalid Claim No. 981634 Samuel J. REED, Company B 115 Regt. Ohio Inf. Vols. On this 6th day of February, A.D. 1897, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Thaddeus J. FLOHR aged 52 years, a resident of Canton in the County of Stark and State of Ohio, well known to me to be reputable and entitled to credit, and who being duly sworn, declares, in relation to aforesaid case, as follows: That he became acquainted with claimant Samuel J. REED as comrades of Co B 115 Ohio Inf. Vols. Was in the same Block House (No. 6) on Stewarts Creek Tenn in summer of 1864 with Lt. PEIRCE in command and I was sergt. And about July 64 Lt. PEIRCE left the Block House in my charge and we held it until the WHEELER raid last of Aug 1864 when we were captured and parolled and during the time above stated S.J. REED was suffering with camp diarrhea and several of them had the same which we laid to the use of the creek water which was all that we could get that was hauled to the Block House and put in a cistern for use. We had no physician at the Block House and we had to send Murfreesboro for medicine and when anyone got very sick he was sent to Murfreesboro. Comrade REED remained with us and did duty until captured although suffering with above named trouble. He further declare that he has no interest in said case and is not concerned in its prosecution. REED, Company B 115 Regt. Ohio Inf. Vols. On this ___ day of _____, A.D. 1891, personally appeared before me, a Notary Public in and for the aforesaid County, duly authorized to administer oaths, Henry P. CORMANY aged 52 years, a resident of Canton in the County of Stark and State of Ohio, well known to me to be reputable and entitled to credit, and who being duly sworn, declares, in relation to aforesaid case, as follows: That he became acquainted with the applicant S.J. REED as a comrade in Co B 115 Ohio and served with him nearly all of the time of service. Was with him at Block House No. 6 in Stewarts Creek Tenn in summer 1864 and at that place we were compelled to use creek water which was hauled to quarters and stored in a wooden cistern in the Blk House and we nearly all suffered from camp diarrhea which we attributed to the use of the water - this applicant suffered from diarrhea and at the time of our capture there last of Aug '64 he was weak from this trouble. After being parolled we all marched to Nashville, Tenn. and from there to Murfreesboro, Tenn. We were together at Christiana, Tenn. in winter and spring of 1865 except a few weeks where applicant was in the hospital and remember distinctly that comrade REED was not well then looked bad being much reduced in flesh. We came home to Canton, Ohio in summer of 1865 and soon after getting home learned of him that he was suffering with piles. Have known him well ever since our return here and know that he has never fully recovered from his bowel trouble. Have seen him on an average of once a week and from the appearance of the man and my conversations with him know the above facts to be true and would place his disability for manual labor thereby at least one half. CORMANY GENERAL AFFIDAVIT State of Ohio, County of Stark In the matter of Original Invalid Claim No. 981634 Samuel J. REED, Company B 115 Regt. O. Vol. On this 6th day of March, A.D. 1891, personally appeared before me, a Clerk of Courts in and for the aforesaid County, duly authorized to administer oaths, James J. CLARK aged 51 years, a resident of Canton in the County of Stark and State of Ohio, well known to me to be reputable and entitled to credit, and who being duly sworn, declares, in relation to aforesaid case, as follows: I was first Sergt. Co. B 155th O.V.I. in Summer of 1864 and had known Samuel J. REED from the time of his enlistment into Co. B in August 1862. At that time and up to Summer of 1864 I regarded him as the most rugged and efficient soldier in the Company. He was not under my immediate Command in Summer of 1864, but I know he was at Blockhouse No. 6 on N&M RR Tenn, as I was then at Block House No. 5 on same road, and the two were only about two miles apart. No. 6 was under the Command of a Sergt. And on the last day of August 1864 was struck by WHEELER's Cavalry and captured. REED was among the prisoners and on the second day after capture he and his comrades were paroled and returned to Murfreesboro, Tenn., where he remained in Garrison till after December 1864. From Sept 1, 1864 till he was sent down the road below Murfreesboro I saw more of him. He was then suffering from diarrhea and piles, and was much of the time unfit for duty, and only did garrison duty. He refused to go to the Hospital though urged to do so. He continued in much the same condition till the Close of the service. He was sober and correct in all his habits and I can only think his sickness and subsequent disabled condition was referable to exposure and bad water and bad food in the service. CLARK Late 1st Sergt. Co B 115 O.V.I. PHYSICIAN'S AFFIDAVIT State of Wisconsin, County of Rock, ss: Of Samuel REED, late of Co. B 115 Ohio Inf. Personally came before me, a Notary Public in and for the aforesaid County and State H.B. JOHNSON, a citizen of Beloit, Wis. Whose Post Office address is Beloit, Wis., well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to aforesaid case as follows: That he is a practicing Physician, and that he has been acquainted with said soldier while in the Army and that he knows that he was a sound man when he enlisted in the above Co. and Regt. In the winter of 1864 after the Battle of Nashville, said soldier came to the Regt. At Murfreesboro reporting his escape from the Rebel Army, his entire detachment being captured in one of the Block Houses between there and Nashville and were started for Andersonville but he succeeded in getting away from the escort. On his return to Camp he had hemorrhoids severely and stated that he had "camp diarrhea" in the Block House which brought on the piles. He was treated in Quarters mainly by my asst. Surgeons JONES and REED. Department of the Interior BUREAU OF PENSIONS Washington, D.C., July 20th, 1891 SIR: It is alleged that Samuel J. REED enlisted Aug 11th, 1862 and served as a Pvt. In Co. B, 115 Reg't Oh.Vol.Inf. and was discharged at Murfreesboro, Tenn., June 22, 1865. It is also alleged that while on duty at Block House No. 6, Nashville and Chattan. In case of the above-named soldier the War Department is requested to furnish an official statement of the enrollment, discharge, and record of service so far as the same may be applicable to the foregoing allegation together with full medical history. That he was treated by Dr. Aaron GRAVES of Navarre, Stark Co., Ohio who died there about twenty years ago and then by Dr. L.M. WHILTING of Canton, Ohio who died here some ten years since. I remember well that he was frequently laid up for several days at a time with Bowel trouble. These troubles continued during all the time he had his home with me.
