On this Sixteenth day of October, A.D., 1865, personnaly appeared before me, Judge of the Probate Court, a Court of Record within and for said county, Edward R. Porter, aged 18 years, a resident of Madison Tp, in the county of Williams, and State of Ohio, who, being first duly sworn according to law, declares that he is the identical Edward R. Porter, who enlisted in the service of the United States, at Pioneer, in the County of Williams, and State of Ohio, on the Seventh day of December, in the year 1863, as a private in Company "G", commanded by captain Joseph Ice, in the 68th Regiment of Ohio Volunteer Infantry, commanded by Lt. Col. Wells in the war of 1861, and was honorably discharged on the Tenth day of July A.D. 1865, as will appear by his certificate of discharge herewith presented. That while in the service aforesaid, and in the line of his duty, on the 20th day of August, A.D., 1864 at Atlanta while advancing with ___ skirmish line of said ___ he received a gunshot wound in the left shoulder-The ___ ____ at the back part of the shoulder at the _____ of the shoulder blade-fracturing the shoulder blade at the _____--thence _____ through to the left breast near the heart and then lodging where it still remains. Rendering the shoulder joint totally useless, and making it impossible to move the arm above the elbow, and causing a great stiffness of the left arm and fingers and makes the arm allmost entirely useless for any purpose of manual labor-rendering the applicant unable to gain a subsistence by manual labor. That since leaving the service he has resided at Madison Township, in the State of Ohio, and his occupation has been nothing. He makes this declaration for the purpose of being placed on the Invalid Pension Roll of the United States, on the account of disability above stated.
