days, personally appeared before me a Notary Public within and for the 
county and state aforesaid, JOSEPH CROCKETT GULLETT, aged 48 years, a 
resident of Moore, rt3, box #28, county of Cleveland, state of Okla... 
makes the following declaration in order to obtain reimbursement from the 
accrued pension for expenses paid by claimant for the last sickness and 
for the burial of WILLIAM THOMAS GULLETT, pensioner of US by certificate 
# 590113 , last paid to aug.4, 1921 by the pension office at wash. dc.
