The Dalles Chronicle, March  31, 2005
     Richard Stephen Forman, 56, a lifetime resident of Madras, died at a 
Madras hospital on March 27, 2005.
     He was born July 8, 1948 in The Dalles to Roy and Phyllis Forman. 
     He married Linda (Damberger) Forman on April 15, 1970 at Reno, Nev.
     He served in the U.S. Army between the years 1968 to 1970. Prior to his 
illness he was a self-employed as a truck driver.
     He was a member of the Madras VFW Post as well as St. Patrick’s Catholic 
Church in Madras.
     He enjoyed trucks and hunting and being with people.
     Survivors include his wife, Linda, Madras; son and his wife, Steven and 
Sandy Forman, Madras; daughters and their husbands, Niki and Brent Casiglio, 
Colleyville, Texas; his mother, Phyllis Forman, The Dalles; brothers and their 
wives, Roy “Sonny” and Pearl Forman, Silverlake; and Lowell and Molly Forman, 
Antelope; sisters and their husbands, Carolyn and Jon Wood, The Dalles; Gwen 
and Richard Johnson, The Dalles; Sharon and Edward Bystran, Aplos, Calif. and 
Marilyn and Robert Senko, Banks; and two grandchildren.
     He was preceded in death by his father.
     A memorial service will be held on Monday, April 4, at 1 p.m. at St. 
Mary’s Catholic Church in Antelope. Final disposition was private.
     Memorials may be made the M.S. Foundation.
     Bel-Air Colonial Funeral Home of Madras was in care of arrangements.
