Ainsworth of Pittsburgh, mother and surviving parent of Elroy Ainsworth, born July 4, 1885. He will not go to school and he will not work. He keeps company with young men of bad habits, shooting craps, and other dissolute habits. He will not obey me and I am afraid he will get into serious trouble. 361 Soho Street Sylvia E. Downing, sister of Elroy Ainsworth, corroborates the above statements. 361 Soho Street Additional Comments: Notes made upon arrival at Pennsylvania Reform School, Morgnza, Pa.: Elroy Ainsworth, #5679, Division D. Father dead. Mother Elizabeth A. Ainsworth, a canvasser, living at 361 Soho St., Pittsburg, Penn. Sent here for not going to school, disobedience, etc. Description: Height 5'1", weight 107 pounds. Brown hair, gray eyes, florid complexion, pug nose. Bicycle wheel with wings & banner with 1898 in India Ink on left forearm. Age 14 on July 4, 1899; received Apr. 19, 1899. Born Penn., American parentage. Attends Centenary church. Mother not intemperate, owns property. 3 older brothers, 1 older sister. Can read and write. Swears and lies; does not steal, use tobacco.
