The father of the infant, who is under the age of 16 years, has been dead for eleven years. Uncontrollable. Will not go to school, stays out at nights, uses vile and profane language. Residence: 50 Cedar Ave., Allegheny. R.B. and Sally Foley of 49 Cedar St., Allegheny, testified the same. Additional Comments: Notes made upon the boy's arrival at Pennsylvania Reform School at Morganza: Herman Flack, No. 5399, Division F. Father dead. Mother, Elizabeth Flack, living at 234 Pacific Ave., Pittsburgh. Formerly at 50 Cedar Ave., Allegheny. Sent here for not going to school & staying out at night. Description: height, 5'5"; weight, 110 pounds. Dark brown hair, dark gray eyes, dark complexion, pit on right side of nose. Two vaccine marks on left arm. Has a right scrotal rupture. Age: 16 on Dec. 1, 1897; received Oct. 21, 1897. Born in Pa. of German parentage. Presbyterian. Neither parent intemperate; parent owns property. 1 older brother, 1 older sister. Can read and write. Swears, lies, uses tobacco, is intemperate. Does not steal. Has not been arrested before. Also enclosed in the docket folder: Iron City College, Pittsburgh, Sept. 20, 1897. The bearer, Mr. Herman Flack, is a student in regular attendance at this Institution, and resides at 50 Cedar St., Allegheny. (signed) Chas.? J. Smith, Principal.
