(J) MACKEY aged 51 years, a resident of ALTOONA in the County of BLAIR and State of PENNSYLVANIA whose Post Office Address is #429-8th Ave, Altoona, PA well known to me to be reputable and entitled to credit, and who, being duly sworn, declared in relation to the aforesaid case as follows: JUNE 1885 I filed an application for a pension on account of DIZZINESS of HEAD and LOSS of SPEECH at times and general debility, being the result of a SUN STROKE which I received about JUNE 15TH OR 16TH, 1863 while on a march from FAMOUTH, VA to GETTYSBURG, PA. I now amend my claim and claim a pension of account of such SUN STROKE and resulting in debility, nervous prostration, mental derangement, vertigo, disease of brain, disease of spine, disease of heart, and impairment of eye sight. MACKEY a resident of ALTOONA in the County of BLAIR and State of PENNSYLVANIA whose Post Office address is #429-8th Ave, ALTOONA, PA well known to me to be reputable and entitled to credit, and who, being duly sworn declare in relation to the aforesaid case as follows: I was a member of COMPANY "H", 4th REGIMENT, PENNSYLVANIA VOLUNTEERS from APRIL 1861 to JULY 1861, so I was in no military or naval service of the United States prior to APRIL 1861 nor since JUNE 1865 and I was in no service between JULY 1861 to AUGUST 1862. I am not able get medical evidence as to treatment for SUN STROKE while in the service for the reason that Dr. JOHN W. ALLEN, Asst. Surgeon of the 148 PA VOLUNTEERS who treated me while in the service died last July or August 1895. He was a member of the PA VOLUNTEERS Board of Pension Examiners at the time of his death in Altoona, PA. I was in the presence of T.(Theophilus) H. Nicewonger of Altoona, PA December 4, 1895, and he wrote this my affidavit as I dictated the same to him to write and I was not prompted nor aided in so doing. WILLIAM I. MACKEY (Signatures of Affiants) CLAIMANTS AFFIDAVIT STATE OF PENNSYLVANIA, COUNTY OF BLAIR, SS: In the matter of my pension claim for pension under the general law, as of COMPANY "F", 148TH REGIMENT, PENNSYLVANIA VOLUNTEERS, on the 5th day of JUNE, A.D. 1897, personally appeared before me an Alderman in and for the aforesaid County, duly authorized to administer oaths, WILLIAM I. MACKEY a resident of ALTOONA in the County of BLAIR and State of PENNSYLVANIA whose Post Office address is #429-8th Ave, ALTOONA, PA well known to me to be reputable and entitled to credit, and who, being duly sworn declares in relation to the aforesaid case as follows: When I entered said Service I was a carpenter and when I was discharged from said Service I suffered with the affliction of SUN STROKE, and about the latter part of JULY 1865 I went to work at my said occupation, but being in the sun it so prostrated me that I had to quit work, and then began working in the Planing Mill, but it was very warm in the Mill I had a throb in my head and it would cause a nervousness. In 1865 when I was discharged from said Service I resided in MILESBURG, PA and each year until 1870 and my post office address was the same. Then I moved to ALTOONA, PA and have been here ever since EXCEPT one year, and my address has been ALTOONA, PA. The ONE YEAR I was away from ALTOONA, PA I was at MILESBURG, PA and my occupation since being discharged was that of a making doors in a planing mill. The SUN STROKE I received while on the march from FALMOUTH, VA to GETTYSBURG, PA JUNE 15-16, 1863. The same results have continued to get worse ever since to this present time. First Dr. George Hale treated me while at MILESBURG, PA in about AUGUST 1865 but he is dead, he died at MILESBURG, PA. Dr. Potter treated me also off and on as Dr. Hale had from 1865 to 1870. They are both dead. Dr. Potter was from Bellefonte, PA. Dr. Bell treated me since the time after I came to ALTOONA, PA and the doctor died. Then Dr. Bulick began treating me and so on until he moved to Harrisburg, PA where he is yet. Dr. Bulick's affidavit is filed, and Dr. H. H. Rush of Altoona, PA also treated me, but he is dead. And by reason of the SUN STROKE and its results he has been, as near as he can give from date of his disability to this date, one half disabled from performing his usual occupation and would not have been able at all to work at manual labor, where it required any heavy exertion and more particular in the sun. TAYLOR aged 57 years, a resident of ALTOONA, PA in the County of BLAIR and State of PENNSYLVANIA whose Post Office address is #507-5th Ave, Altoona, Pa., well known to me to be reputable and entitled to credit, and who, being duly sworn, declare in relation to the aforesaid case as follows: I was a Sergeant of COMPANY "B", 148TH REGIMENT, PENNSYLVANIA VOLUNTEERS INFANTRY from about AUGUST 1862 until about JUNE 15, 1865, and I was personally acquainted with said WILLIAM I. MACKEY prior to the late rebellion, and while we were on the march from FALMOUTH, VA to GETTYSBURG, PA in the month of JUNE 1863, said Claimant and I met, at which time we were both exhausted and played out. I suffered with a chill and Claimant suffered with a NERVOUS PROSTRATION, THE RESULT OF A SUN STROKE, and shortly after we were discharged I met Claimant, and he still suffered from with NERVOUSNESS AND WEAKNESS as the result of SUN STROKE, and I have met and conversed with him each year from 1865 to this date, and during the same time he suffered the same trouble, and during the same years in my honest opinion he has been entirely disabled from earning a support at manual labor on account of said disabilities during each year from 1865 to this date JULY 1896. GETTEMY, M.D. 400-4th Ave, Altoona, PA January 3, 1925 To Whom it may Concern: This is a supplementary report on the physical condition of W. I. MACKEY, 210- 4th Ave, Altoona, PA. He first consulted me in March of 1923, at that time his mental condition was good. His chief difficulty at that time was an INTENTIONAL TREMOR affecting all parts of his body, head, arms and legs. This was most marked in the ARMS and HEAD. He was unable to care for himself in such acts as spreading bread, carving meat, getting any receptacle containing liquid to his mouth, that he might drink. He was able to get his clothing on but unable to fasten them, because of being unable to do these acts he has required constant attendance. This condition is progressive therefore, he is going to require constant attendance the rest of his days. He also suffered with RHEUMATISM in both FEET, LEFT KNEE and RIGHT HAND. I have about the same trouble in eating and drinking and sometimes to be held, while the cup is put to my lips on days that the RHEUMATISM is bad. I am the Member of Post Dewey Camp -----. I agree, if flag is issued, to comply with Par. 2 of Instructions on this form. I certify that to the best of my knowledge and belief the statements made above are correct and true; that a flag has not been previously applied for or furnished for the above deceased veteran; and that I have carefully read Paragraphs 1 to 3 of the Instructions and that this application is not submitted in violation of Section 35 of the Criminal Code, which provides a fine of not more than ten thousand dollars, or imprisonment for not more than ten years, or both, for presenting any claim against the Government of the United States, knowing said claim to be false with intent to defraud. They had 11 children some children were born in Milesburg, Centre County, PA and some children were born in Altoona, Blair County, PA. WILLIAM & CANDACE moved to Altoona, PA around 1870. WILLIAM I. MACKEY was sometimes written as WILLIAM J. MACKEY probably due to the handwritten capital "I" and capital "J" which looked similar when written on unlined paper. WILLIAM I. MACKEY's Civil War Record: 4th Regiment, Company "H", PA VOLUNTEERS, Infantry Enlisted: April 19,1861 at Bellefonte, Centre Co, PA and Mustered Out: July 26, 1861 at Harrisburg, PA. ARMY OF THE POTOMAC, 148th Regiment, Company "F", PA VOLUNTEERS, Infantry, Enlisted in Milesburg, Centre Co, PA on August 13, 1862 and Mustered Out with his Company on September 9, 1865 at Alexandria, VA. On June 15-16, 1863 had sun stroke while on a forced march from Falmouth, VA to Gettysburg, PA. On May 13, 1864 he received a gun shot wound in the left leg at Spottsylvania, PA WILLIAM I. MACKEY fought in the Battle of Gettysburg. On the Monument of the ARMY OF THE POTOMAC, 148th Regiment, Company "F", Infantry, PA Volunteers, near the Wheatfield on the battlefield at Gettysburg his name appears as WILLIAM J. MACKEY, Corporal. His brother MARTIN H. MACKEY is listed in the same Company as a Sergeant. WILLIAM IDDINGS MACKEY & MARTIN H. MACKEY's father was JOHN MACKEY born Franklin County, PA in 1802. JOHN MACKEY served in the Civil War in the 26th Regiment, Company "C", PA Emergency Militia Infantry. Enlisted June 19, 1863 and Mustered Out July 30, 1863. The 26th Regiment was organized at Harrisburg, PA for the protection of Pennsylvania against Lee's invasion. WILLIAM IDDINGS MACKEY died February 26, 1933 and is buried in Fairview Cemetery, Altoona, Blair Co, PA. MARTIN H. MACKEY died April 11, 1913 and is buried in Fairview Cemetery, Altoona, Blair Co, PA. JOHN MACKEY died August 17, 1886 and is buried in Fairview Cemetery, Altoona, Blair Co, PA. ACT APPROVED MAY 1, 1920 DECLARATION FOR PENSION This form is only to be used by or in behalf of one who desires to claim original pension or under section 2 of the Act of May 1, 1920, because he requires the regular personal aid and attendance of another person. The declaration and testimony in support thereof should be executed before some officer authorized to administer oaths for general purposes. ACT APPROVED MAY 1, 1920 Section 2 reads as follows: That every person who served ninety days or more in the Army, Navy, or Marine Corps of the United States during the CIVIL WAR, and who has been honorably discharged therefrom, or who having so served less than ninety days, was discharged for a disability incurred in the service and in the line of duty, or is now upon the pension rolls as a CIVIL WAR Veteran, and every person who served sixty days or more in the WAR with MEXICO, or on the coasts or frontier thereof, or en route thereto, during the war with the nation and was honorably discharged therefrom, and who is not, or hereafter may become, by reason of age and physical or mental disabilities, helpless or blind, or so nearly helpless or blind as to require the regular personal aid and attendance of another person, shall be entitled to and shall be paid a pension at the rate of $72 per month. INSTRUCTIONS If the applicant claims that, by reason of age and physical or mental disabilities he is helpless or blind, or so nearly helpless or blind as to require the regular personal aid and attendance of another person, he should file with his application. The sworn statement of the attending or family physician, describing the disabilities which require the regular personal aid and attendance of another person, and giving the date from which such aid and attendance has been required; or, if the claimant is unable to procure such statement- The sworn statement of the claimant's attendant showing the character and frequency of the aid and attendance required, and from what date; whether the claimant is confined to the house or to his bed and, if so, whether for the whole or only a portion of the time; and the relationship existing between the attendant and the claimant. READ CAREFULLY Under the law a person may not receive pension for the Bureau of Pensions and compensation or vocational training pay through the United States Veterans Bureau covering the same period of time. That part of the declaration referring to service between April 16, 1917, and February 9, 1922, should show whether the claimant or any member of his family rendered any service in the Army, Navy, or Marine Corps. of the United States during said period, and if so, the full name under which each such member served should be stated, together with the designation of the organization in (or the vessel on) which such service was rendered, with dates of enlistment and discharge. The term "family" includes: Child, legally adopted child, stepchild, father, mother, stepfather, stepmother, father and mother through adoption, and person who stood in place of parent for a period of not less than one year prior to induction into service.
