DECLARATION FOR PENSION THE PENSION CERTIFICATE SHOULD NOT BE FORWARDED WITH THE APPLICATION. STATE OF Pennsylvania, COUNTY OF Cambria, SS: On this 6th day of November, A.D. one thousand nine hundred and twelve, personally appeared before me, a Notary Public within and for the county and State aforesaid, William H. Rose who, being duly sworn according to law, declares that he is 74 years of age; and a resident of City of Johnstown, county of Cambria, State of Pennsylvania; and that he is the identical person who WAS ENROLLED at Harrisburg, Penna., under the name of Wm. H. Rose, on the 31st day of January, 1862, as a 1st Lieutenant & Adjutant, in Fifty- fourth Regiment Pennsylvania Volunteers in the service of the United States, in the Civil war and was HONORABLY DISCHARGED at Harper's Ferry, Virginia, on the third day of September, 1864. That he also served 2nd Lieutenant of Co. I Fifty-fourth Pa. Vols. from 6 Aug. 1861 and as such mustered into U.S. serviced Oct. 10th, 1861, and served in such rank until promoted to Adjutant Jan. 1, 1862. That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, five feet ten inches; complexion, light; color of eyes, grey; color of hair, light; that his occupation was lawyer; that he was born November 17th, 1838, at Johnstown, Cambria County, Pennsylvania. That his several places of residence since leaving the service are as follows: Johnstown, Cambria County, Pennsylvania. That he is a pensioner under Certificate No. 96452 reissued. That he has not applied for pension under original No. . . . That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912.
