                    APPLICATION FOR EMPLOYMENT.
  USE INK.  To be filled by applicant in his own handwriting.  Applicants under 
  21 years of age must furnish minor's release (Form 265) with each 
  application.  All statements to be made under oath.
  
  It is expressly understood that the period of the service of the applicant may 
  be terminated at any time by this Company and pay allowed only for the actual 
  time worked, regardless of whether applicant by employed by the month, week, 
  day or hour.
  
    The applicant will only be considered as in the temporary service of this 
  Company until his application shall have been approved by the superintendent 
  of the Employment Bureau.

                         KNOW ALL MEN BY THESE PRESENTS:
  That we, Charles A. King and Emma M. King (parents) of Mabelle King a minor, 
  under the age of twenty-one (21) years, do hereby request, and consent to, the 
  employment of said minor by Erie RAILROAD COMPANY, and, in consideration of 
  such employment, do hereby agree that the wages, which shall be payable by 
  reason thereof, shall be paid to him; and we (I or we) do hereby, in 
  consideration of such employment and the wages so to be paid, release and 
  forever discharge said Erie RAILROAD COMPANY, and all other lines or 
  companies, teased, operated or controlled by or allied with it, and each of 
  them, together with their and each of their successors and assigns, from any 
  and all liability for wages so paid to said minor, and from any and all 
  liability growing out of the death of, or any injury to the said minor, 
  occurring while in the employment of said Erie RAILROAD COMPANY, whether 
  caused by the negligence of said Erie RAILROAD COMPANY, or of any of its 
  officers agents servants or employees or otherwise.
