April 17th, 1867 Sir: I have the honor to acknowledge the receipt from your Office of application fo rPension No. 122.564, and to return it herewith, with such information as is furnished by the files of this Office. It appears from the Rolls on file in this Office, that Frank Jacobs was enrolled on the 28th day of Feby, 1862, at Blair Co., in Co. L 12th Regiment of Pa. Cavy Volunteers, to serve 3 years, or during the war, and mustered into service as a Saddler on the 5th day of March 1862, at Phila Penna, in Co. L, 12th Regiment of Penna Cavy Volunteers, to serve 3 years, or during the war. On the Muster Out Roll of Co. L of that Regiment dated March 22 1865, he is reported a "Pvt" mustered out with his company on that date. Previous rolls report him sick in hospital. If such officer uses a seal, certificate of Clerk of Court is not necessary. If no seal is used, then such certificate must be attached. State of Penna County of Erie On this 29th day of June A.D. one thousand eight hundred and ninety two, personally appeared before me, a Prothonotary within and for the County and State aforesaid, Frank Jacobs, aged 51 years, a resident of the City of Erie County of Erie State of Penna, who, being duly sworn according to law, declares that he is the identical Frank Jacobs who was enrolled on the 6th day of February 1862, in Sadler Company L 12th Regt Penna Vol Cavalry for three years serving in the war of the rebellion, and served at least ninety days, and was honorably discharged at Harpers Ferry, Va., on the 22nd day of March 1865. That he is now unable to earn a support by manual labor by reason of gun shot wound left hand acquired in action at Halltown, Va June 29th/64 causing loss of little finger at middle joint and loss of use of said left hand, chronic diarrhea and general kidney & liver trouble as contracted while in service three years 1862 to 1865 cannot lift anything with injured left hand. That said disabilities are not due to his viscious habits and are to the best of his knowledge and belief permanent. That he has _____ applied for pension under application No. _____. That he is a pensioner under Certificate No. 86.330 at $4 per month entirely to serve for actual disability? That he has not been employed in the military or naval service otherwise than as stated above. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of June 27, 1890. He hereby appoints John L. Wells of Erie, Pa. his true and lawful attorney to prosecute his claim, and he directs the sum of ten dollars to be paid to said attorney. Sheriff(?) persons whom I certify to be respectable and entitled to credit, and who, being by me duly sworn, say they were present and saw Frank Jacobs, the claimant, sign his name (or make his mark) to the foregoing declaration; that they have every reason to believe from the appearance of said claimant and their acquaintance wtih him for 25 years and 25 years, respectively, that he is the identical person he represents himself to be; and that they have no interest in the prosecution of this claim. State of Panna County of Erie On this 6th day of March, A.D. one thousand nine hundred and seven, personally appeared before me, a Notary Public within and for the cuonty and State aforesaid, Frank Jacobs, who, being duly sworn according to law, declares that he is 65 yeas of age, and a resident of City of Erie county of Erie, State of Penna; and that he is the identical person who was enrolled at _____ City Pa under the name of Frank Jacobs, on the 5 day of March, 1862 as a Private, in Company L 12th Regt Pa Vol Cavalry in the service of the United States , in the Civil war, and was honorably discharged at Harpers Ferry, Va., on the 22nd day of March, 1865. That he also served - none - That he was not employed in the military or naval service of the United States otherwise than as stated above. That his person description at enlistment was as follows: Height 5 feet 9 1/2 inches; complexion light; color of eyes, blue; color of hair, light; that is occupation was sadler; that he was born May 23, 1841 at Chautauqua County. That he is ____ a pensioner. That he has _____ heretofore applied for Pension Certficate No. 86330 at ten dollars. The he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of February 6, 1907.
