S. Surgeon at time of enlistment and was pronounced sound - and that he was sound and free from rupture at the time of enlistment but he was ruptured on the 12" day of March 1863 on the march from Newburn to Pellicksville on said day he was jumping across a ditch on the time of our march The regiment physician I understand is not now living The physician that attended me after I was discharged - Dr. Kennedy at Orestown Franklin County Pennsylvania I cannot find I have written to Orestown but only find that he left there and went to Kansas - soon after he attended me - and I have been unable to find him after writing to various places and making inquiry in various ways - John (his) X (mark) Reynolds Attest - L. Knoll of the county and state aforsaid who I certify to be entitled to credit and who being duly sworn according to law does depose and say that he was well acquainted with John Reynolds who was a private in Co. B. 158 Regiment Pennsylvania Volunteers that he knows from personal observation that said Reynolds was ruptured in the right groin that said Reynolds showed him the rupture while at Newbern in North Carolina and he further says that his knowledge of the above facts are as stated above and he believes that said Reynolds is entitled to a pension and that he has no interest direct or indirect in the prosecution of his claim G. He says verry likely my claim is just but his memory is to short to remember the particulars in my case the facts are that by his Dr N. G. Leet assistant Surgeon 158 Pa Vol did in the forepart of June 1863 order and direct me to the hospital at Fort Monroe for Rupture in right groin and I have been unable to do manual labor ever since by reason of said Rupture John Reynolds Harrisonville Cass Co Mo _____________________________________________________________________________________ Harrisonville Mo June 13th 1887 Hon Commissioner of Pensions Dear Sir I have been informed by C. My statement to the above is that I never served in any capacity except in Co B 158 Reg Pa Vol Inft you have affidavits in my claim for pension in your office of said service I also wish to refer you to a letter written by me on the 16th of April 1885 revoking all Power of attorney of C S McArthur in the procecution of my claim for pension and requesting all communications to be addressed to Me Very Respectfully John Reynolds Harrisonville Cass Co Mo _____________________________________________________________________________________ State of Nebraska County of Polk ss: John W. Detwiler of the County and State afore said being duly sworn according to law does depose and say that he was well acquainted with John Reynolds of Company B 158 regiment Pennsylvania Volunteer Infantry that he was with him and belonged to thew same company and regiment and in the fore part of June 1863 said Reynolds was sent from Hollys Landing Virginia to Fortress Monroe Hospital by reason of Rupture in the right groin and he further says that he saw said Reynolds in Hospital and knows from personal knowledge that said Reynolds was ruptured as above stated and he further says that he has no interest directly or indirectly in the procecution of his claim for pension Claim No 492928 John W. (form) Under an act granting pensions to soldiers and sailors who are incapacitated for the performance of manual labor and providing for pensions to widows, minor children, and dependent parents. ------------------------ State of Illinois County of McLean ss: On this 8th day of July, A. D. one thousand eight hundred and ninety, personally appeared before me, Wm L Vockrodt, a Notary Public in and for the county and state aforesaid, John Reynolds, aged 59 years, a resident of Danvers, County of McLean, State of Ills. , who being duly sworn according to law, declares that he is the identical John Reynolds, who entered service during the War of the Rebellion under the name of John Reynolds on or about the 16th day of October, 1862, as private in company B of the 158 regiment of Pa vols commanded by Captain lamon and was HONORABLY DISCHARGED at Chambersburg, on or about the 12 day of August, 1863, by reason of time expired; that his personal description is as follows: Age, 31 years height 5 feet 10 1/2 inches; complexion, light; hair Dark; eyes Blue. That he is now suffering from rupture right groin and that the said disability is of a permanent character, and is not the result of vicious habits, and that it incapacitates him from the performance of manual labor in such a degree as to render him unable to earn a support, and that this declaration is made for the purpose of being placed upon the pension roll, under the provisions of the Act of June 27, 1890. That he has not been employed in the military or naval service otherwise than stated above Infantry That since the 12th day of August, A. D. 1863, he has not been employed in the military or naval service of the United States. He hearby appoints, with full power of substitution and revocation, GEORGE E. LEMON, Of Washington, D. C., his true and lawful Attorney, to prosecute his claim. That he has never received but has applied for a pension. Who being duly sworn according to law states that, My rupture on left side has been coming on gradually for about nine years. The Board of Examiners at Bloomington pronounced my disease throat, lung and heart trouble on Oct 28th 1891. I knew for several years there was something wrong and was treated by Doctor Parkhurst of Danvers, Ill but did not know what was the matter until examined by the board on Oct. 28th 1891. My disabilities are not due to vicious habits, and are to the best of my knowledge and belief of a permanent character. George E. Lemon is not my attorney on this new claim. D. one thousand eight hundred and ninety - five, personally appeared before me, a Notary Public of the - within and for the county and State aforesaid Emma Reynolds, aged - years, a resident of the - of - County of McLean, State of Illinois, who being duly sworn according to law, declared that she is the widow of John Reynolds, who was drafted under the name of John Reynolds at Chambersburg, on the - day of - (I have not got the date of the time he was drafted) A. D. 1862, in Private Co B 158 regiment Pennsylvania Drafted Militia Infantry and served at least ninety days in the late War of the Rebellion, in the service of the United States; who was HONORABLY DISCHARGED - , and died December 4th 1895. That the soldier was not in the Military or Naval service of the United States except as above stated. That she married under the name of Emma Souder, to said John Reynolds, on the 17th day of December 1874, by Samuel Zook, at Orriston Penn there being no legal barrier to said marriage. John Reynolds first wife Matilda Reynolds Died Dec 1st 1872 as for family ?respond? enclosed. That she has not remarried since the death od said -. That she is without other means of support than her daily labor. That names and dates of birth of all the children now living under sixteen years of age of the soldier are as follows: Harry C. , born April 8th, 1881 Cora O. , born May 30th, 1884 That she makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of June 27, 1890. She hearby appoints -, of - State of -, her true and lawful attorney to procecute her claim and receive a fee of $ -. That her post office is Danvers county of McLean, State of Illinois.
