Your petitioner therefore applies, by petition in writing, to the said Honorable Court and prays that your Honours will grant him such relief as is prescribed by the several Laws of Insolvency of this commonwealth; and as in duty bound he will ever pray & c. The within-named petitioner, being Sworn according to law, declared and says that the foregoing is a just and true “Schedule of all the property, real, personal, and mixed, to which he is in any manner entitled” and that the “List of his Creditors, and the nature and amount of their, as far as he can ascertain the same,” and the “Statement of his Losses, and the means whereby he became insolvent,” exhibited with and annexed to this petition, are likewise true, just and correct, to the best of his knowledge and belief.
