                          State of Pennsylvania

On this twenty four Day of August in the Year One Thousand Eight Hundred and 
four Before me Benjamin Nones, Esquire, Notary Public for the Commonwealth of 
Pennsylvania, duly commissioned and authorized by Law to administer Oaths and 
Affirmations, dwelling in the City of Philadelphia, personally came John 
Norris a Citizen of the United States of America, who being duly Sworn 
according to Law, on his solemn Oath doth declare and say, that he was born in 
the City of New York in the State of New York in the said United States, and 
that he is thirty Years of Age, and that his ____________ are now living and 
actually reside in the State of __________  and that having shipped himself on 
board of the ___________________ of ___________ commanded by Captain 
_________________ on a Voyage to _______________ he therefore desires of me 
the Notary aforesaid to describe his Person, which I find to be of the height 
of five Feet four Inches, Dark brown Hair, Grey Eyes, Common Nose, round 
dimpled Chin, Pock Marked Face, and of a Brown Complexion, a large Scar on his 
left leg, Scar between the thumb & fore finger of his right hand, a Scar over 
his left Eye Brow & a Scar on his tongue.

I do further certify, That at the same Time came and appeared William Johnson 
a Citizen of the United States, who being also duly Sworn did declare and say 
that he is well acquainted with John Norris and verily believes him to be born 
in the City of New York in the State of New York.

                                        my Notarial Seal, the Day and Year
