On this Seventh Day of June in the Year of our Lord One Thousand Eight Hundred 
and Eight Before me George Heyl, Esquire, Notary Public, dwelling in the City 
of Philadelphia, Personally came Thomas Willis of said City Mariner and made 
Oath that he is a Native of America born in Town of Newbern in the County of 
Craven State of North Carolina and a Citizen of the United States that he is 
aged Thirty Years, of the Height of Five Feet Seven & 3/4  Inches black Hair, 
dark Complexion hazel Eyes, sharp visage pitted with the Small Pock, has in 
India Ink Adam & Eve on the right arm and the initials of his name on same 
arm; on the left arm our Saviour on the Cross, and on same arm an anchor and a 
heart.

Joseph McNeall being also duly Sworn made oath that he is well acquainted with 
Thomas Willis and verily believes he was born in Craven County, State of North 
Carolina.
