DECLARATION FOR WIDOW’S PENSION. State of Pennsylvania, County of Philadelphia, SS: On this 5 day of May, A. D. one thousand nine hundred and ________, personally appeared before me, a Notary Public, within and for the County and State aforesaid, Sarah Almond, aged 64 years, a resident of Philadelphia County of Philadelphia, State of Pennsylvania who being duly sworn according to law, declares that she is the widow of Thomas Almond, who enlisted under the name of Thomas Almond on the ______ day of ______, A. D. 18__, as a private in Company “C”, in the 11 Regiment of Penna. Cav. Vol. and served at least ninety days in the late War of the Rebellion, in the service of the United States, who was HONORABLY DISCHARGED ______ and died May 3 1909 That he was ___ employed in the military or naval service otherwise than as stated above _______________________ That he was never employed in the military or naval service of the United States after the ___ day of _____ 18__ That she was married under the name of Sarah Blackburn to said Thomas Almond, on the 30 day of September A. D. 1866 by Rev. Geo. A. Durborow at Phila. Pa, there being no legal barrier to such marriage; that she had not been previously married; that her said husband had not been previously married. That she has not remarried since the death of the said Thomas Almond. That the names and dates of birth of all the children of the soldier, now living, and under sixteen years of age, are as follows: _____________, born ____________, 18__ _____________, born ____________, 18__ _____________, born ____________, 18__ _____________, born ____________, 18__ _____________, born ____________, 18__ _____________, born ____________, 18__ That she has not abandoned the support of any one of her children, but that they are still under her care or maintenance. That no prior application for pension has been filed by herself or the soldier was a pensioner by Cert. 1005608. That she makes this declaration for the purpose of being placed on the pension-roll of the United States, under the provisions of the act of April 19, 1908. She hereby appoints, with full power of substitution and revocation, W. O. Sickel of ___________ her true and lawful attorney to prosecute her claim, the fee to be Ten Dollars, payable as prescribed by law.
