Original Insurance Document

Application of Finley & Hay of Elk Lick in the County of Somerset State of Pa 
for insurance against loss or damage by fire, by the Fayette County Mutual Fire 
Insurance Company of Penna. in the sum of five thousand Dollars for the term of 
three yrs from the 21st day of June 1869, at noon, on the property below 
specified, the value of the property being estimated by the applicant.
Two Stoned Brick House $3,000
Frame & log Barn  $1,000
Hay & Grain in Farm?  $1,000
Am't of Premium Note $480 Premium paid 26.40, Policy and Survey $1.00, Tax $.50

The said applicant make the following statements, and give the following answers 
to interrogatories here put relative to the risk, as a warranty on the part of 
the assured.

House Brick-Barn Wood on applicants farm in Elk Lick Twp. Somerset Co Pa-about 2 
miles South of Salisbury.

Barn about 75 yards from house.

In reply to where are ashes kept?

Carried away from house.

Size of buildings and number of stories
House 48x40 W  Barn 117 x 48 ft.
