Clyde Berkey, the mine foreman, who accompanied Blake and was only a few steps in the rear, miraculously escaped a similar fate. The roof of the entrance had been tested at regular intervals, it is declared and at no time was there any evidence that it had cracked or that there was any danger of a fall. It is said there was not the slightest warning when the roof came down upon Blank, who was buried under several tons of rock and earth; his neck was broken and his back badly crushed. The fall missed foreman Berkey by only a few feet. Mr. Blank was about 28 years of age and with his wife and two children made his home with Mrs. Blake's parents, Mr. and Mrs. L. A. Meyers, who reside about a mile from Hooversville and to whose residence, the remains were taken. He was a brother of George Blake of Hooversville and his sisters reside in New Castle and Kansas. The funeral took place Monday afternoon. Meyersdale Commercial, Oct. 29, 1914
