JOHN HOWARD FORMAN

  John Howard Forman, two-year-old son of Calvin and Dorothy (Mazer) Forman, 
died Monday at the home of his maternal grandfather, Howard Mazer of Sand Patch 
R.D.  Besides his parents and maternal grandparents, he leaves a brother, Jay H.
  Funeral services were held at 2:30 p.m. Thursday at the White Oak Lutheran 
Church.  Interment was made in the church cemetery under the direction of 
Konhaus Funeral Home.
