Hickey, age 57 years, a resident of Baxter, County of Putnam, State of Tennessee, who makes the following declaration as an application for, and claim is hereby made for, reimbursement from the accrued pension for expenses paid (or obligation incurred) in the last sickness and burial of Sandy A. Hickey, who was a pensioner of the United Sates by certificate No. SC-673176, and who died Aug 26 1933 at Baxter, Tenn. Route 2 That the answers to questions propounded below are full, complete, and truthful to the best of my knowledge, information, and belief, and that no evidence necessary to a proper adjustment of all claims against the accrued pension is suppressed or withheld. 1. Hickey, private Company D 1st Regt. Tenn. Mtd. Infantry Vols. Personally came before me, a Justice of the peace in and for aforesaid county and state, Francis M. Anderson of Bloomington of the county of Putnam, State of Tennessee who being duly sworn, declares in relation to the aforesaid case as follows: That he was originally Capt. Company C, 1st Reg. Tenn. Mt. Infantry in the late war of the rebellion and enlisted the Claimant into the Service of the United States and at date of enlistment deponent states that the claimant was sound and healthy so far as he knew or could discovered no visible marks of disease upon him whatever. Deponent further states that the service that 1st Tenn. Mt. Infantry had to do was scouting after bands of Guerillas, that infest the Cany Fork and Cumberland River Hills and the Country was very rough and at times scouts was required to keep nearly constantly on horseback for several days and nights on succession and would thereby become worried to an extent that would cause much complaining of the soldier and in different ways. The applicant Sandy A. Hickey complained of some affliction of the anus or the terminus of the lower gut which deponent supposed was what medical men called piles. Deponent also further states that he has been personally acquainted with the claimant ever since the war and that he claimant has been complaining more or less all the whole of the said affliction. And also that the claimant served his entire enlistment in said company C. and he deponent makes this statement from said acquaintance and service. Francis M. Mtd. Volunteers, was enrolled on the 14th day of December, 1863, at Carthage, Tenn. For 1 year, and is reported: on roll from enrollment to April 30th 1864 present. Same to August 31st 1864, sub rolls not on file. Muster out roll of Co. dated Nashville, Tenn. April 25, 1862. Reports him private mustered out at that date and place. No returns or Co. Morning reports on file. No further information. State of Tennessee County of Putnam In the matter of the application for the pension of Sandy A. Hickey, On this 17th day of August AD 1889, personally appeared before me a Justice of the Peace in and for the aforesaid County, duly authorized to administer oaths, Jacob C. Barr age 57 years, a resident of Burton in the County of Putnam and State of Tennessee, whose Post office address is Burton, Tennessee. That I have been well and personally acquainted with Sandy A. Hickey for 30 years and that I lived in about two miles of him at the time of his entering the Army in 1863, and he was sound man and free from any disability as far as I knew. And that on or about the - day of May 1864 at or near Carthage Tennessee, he was taken with Fistula in anus, on the left side of the Rectum, which disability he has suffered with ever since disabling him from the performance of manual labor to the extent of at least 1/2 of every year since his discharge to the present time. He further states that he has lived a neighbor to said soldier war since his discharge, not more than two miles apart, and that he seen said soldier every few weeks since his discharge and that he was always complaining of his disability, saying that when he tried to work, the affected parts inflamed and became raw so that he could not half work and what he did do was in misery. Said soldier is a farmer by occupation. State of Tennessee County of Putnam In the matter of the application for the pension of Sandy A. Dear Sir, It's now been more than a month since I sent my papers as administrator for Sandy A. Hickey deceased and haven't received his check yet. I can't understand why it has not come. The pension accruing from the date of last payment to the date of death is not an asset of the estate, is not payable to heirs and is payable as reimbursement for the expenses of last illness and burial only if the deceased did not leave assets sufficient to meet such expenses. If the facts in the instant case warrant the presentation of a claim, Form 5036, copy of which is enclosed, completed in accordance with instructions thereon, should be on file in this office within one year from the date of death. In the service of the United States, in the civil war, and was honorably discharged at Nashville, Tenn. on the 25th day of April, 1865. That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, six feet 0 inches; complexion fair; color of eyes, dark; color of hair, light; that his occupation was farmer; that he was born August 9, 1843.
