D. one thousand eight hundred and eighty three personally appeared before me, clerk of the County, a court of record within and for the County and State aforesaid, Ammond D. Jones aged 55 years, a resident of the town of Window Cliffs, County of Putnam, State of Tennessee, who, being duly sworn according to law, declares that he is the identical Ammond D. Jones who was enrolled on the 27th day of September, 1862, in Company G. of the 5th Regiment of Tenn. Cavy., commanded by Capt. Cliff, and was honorably discharged at Pulaski, Tenn. on the 14th day of August, 1865; that his personal description is as follows: Age 55 years, height 5 feet, 6 inches, complexion, fair; hair black,; eyes, Hazel. That while a member of the organization aforesaid, in the service and in the line of his duty at Nashville, in the State of Tennessee, on or about the ____day of December, 1862, he contracted Thypo Malarial Fever which disabled him from service for eight months. He has never recovered and has since been troubled with nervous asthma and rheumatic sufferings. He claims pension on malarial poisoning, nervous asthma and Rheumatism. That he was treated in hospitals as follows: Hospital No. 13- Nashville, Tenn. and Gallitan Tenn. Entered Dec 1862, left July 1863. That he has not been employed in the military or naval service otherwise than as stated above. That since leaving the service this applicant has resided in the town of Window Cliffs in the State of Tennessee and his occupation has been that of a farmer. That prior to his entry into the service the above named he was a man of good, sound, physical health, being when enrolled a ................. That he is now very much disabled from obtaining his subsistence by manual labor by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States. He hereby appoints with full power of substitution and revocation, George F. Davis, of Washington, DC, his true and lawful attorney to prosecute his claim. State of Tennessee Putnam County On this 22nd Day of May personally appeared before me a Justice of Peace in and for the aforesaid county duly authorized to administer oaths, O. D. Harris aged 45 years a resident of Cookeville in the County of Putnam and State of Tennessee and Wm. Massa aged 52 years a resident of Window Cliffs in the County of Putnam and state of Tennessee who being duly sworn according to law state that they are acquainted with Ammond D. Jones applicant for Invalid pension and know the said Ammond D. Jones to be the identical person of that in the 5th US Tennessee Cavy. And Deponents further state that they were well acquainted with claimant having known him for at least 30 years and deponents further state that the claimant was a sound and an able bodied man at and prior to enlistment so far as they knew and had he been unsound we would have heard it. Vol. Cavalry Can No. 5726 Bundle No. 50 State of Tennessee County of Putnam Personally came before me an acting Justice of the Peace in and for aforesaid county and state. Ammond D. Jones whom I certify to be reputable and entitled to credit and who by me first duly sworn according to law, declare as follows: That his place of residence for two years before the war was Putnam County, Tenn., and has been so ever since his discharge. His occupation for more than two years before the war was that of a farmer, and since the war has been that of a merchant. He further states that he was taken down with typhoid malaria fever near Nashville, Tenn., on or about the 1st day of December 1862, and treated by Doctor Moore, Regimental surgeon, (who he learned is now dead), he was then sent to hospital No. 13 Nashville, Tenn., and treated by Doctor Herrix, until about May 1863, when he joined his command at Carthage, Tenn., and remained with his company two or three weeks when he was taken with a relapse and was sent to hospital at Gallatin, Tenn. and remained until some time in July or August 1863. He further states that he has been troubled off and on ever since his return from the army with weakness, chills and fever and rheumatism and that one severe attack of rheumatism lasted at least six months during the year of 1872 and 1873. He further states that he never had nay regular physician as he lived in a remote part from a doctor, but found relief in Ayres Ague Cure as recommended to him by Perry and Demoville, druggists at Nashville, Tenn. And that has treated himself for rheumatism with whiskey and poke root, prickly ash, etc., and by patent medicines and liniments as recommended by above named druggists. He is constantly using patent medicine and has not been able to perform more than 1/4 labor, and that in much pain and distress from the effect of rheumatism and nervous disability. Ammond D. Personally came before me acting Justice of the Peace in and for aforesaid County and state, W. H. H. Grime of Ditty, County of Putnam, and state of Tennessee, and F. M. Clouse of Burton, County of Putnam, state of Tennessee, whom I certify to be reputable and entitled to credit and who made oath in due form of law, that they have known Ammond D. Jones for 30 years past and at the time of their first acquaintance with him he was a stout and able bodied man, and they knew it from the fact that they was with him most every year at Log Rollings, Rail splittings, etc. Until the war commenced, during which time they was not with him. Deponents further states that said Jones returned home from the Army during the month of August 1865, and put up a small stock of goods in his dwelling house. Deponents further states that they were at his house frequently even until at this date, and that said Jones has suffered with rheumatism, chills and fever more or less from date of coming home until the present time, and one very severe attack of rheumatism lasted about six months. Deponents further state that they have lived near him ever since the war and have seen him after every year and that he complained all the while of great weakness and rheumatism, and he has been scarcely able to do any manual labor, and that upon the whole one year after another from the war until the present time the said Ammond D. Jones has been incapacitated for manual labor to the extent of three quarters, and deponents further state that they are not interested in the prosecution of this claim. W. H. H. Grime F. M. M. F. Jones J. P. State of Tennessee Putnam County } Personally appeared before me the undersigned authority, Ammond D. Jones of the State and County aforesaid, and made oath in due form of law that he knew nothing about the whereabouts of any living surgeon who doctored him while in the U. S. Service, as he was ordered to the hospital on the first examination by Doctor More whom he learns is dead. Deponent states that if Dr. Hickman and Dr. Moffit doctored him he did not recollect it and if Doctor Moffit lives in Liberty Tenn. he can't find it out. Ammond D. Jones. State of Tennessee, County of Putnam in this the 30th day of Nov. 1892 personally came before me the undersigned authority duly authorized to administer oaths, C. R. Howell age 20 years as resident of Baxter, State of Tennessee, County of Putnam who being duly sworn says he has been personally well acquainted with Ammond D. Jones for many years as a near neighbor. He has been disabled for manual labor for many years and for the last four years he has been totally disabled for manual labor and has necessarily required the regular assistant to wait upon him all the time. Just the same as a little child dressing him and undressing his person, bringing his meals to him and in a _____all the calls and demands of nature. At some f__________ periods he has by the advice of his physician been carried out to exercise by carriage ride but had to be assisted in and out of the carriage and could not bear to be out longer than from fifteen to twenty minutes at a time. Further affiant says it is his constant business to be at the hand to attend claimant or in his absence another person until he could relieve whomever might be a substitute for a day or night _____ intervals. A. 5th Tennessee cavalry On this 23 day of June, A. d. 1896, personally appeared before me a notary public in and for the aforesaid County, duly authorized to administer oaths, Ira I. Howell age 21 years, whose Post Office address is Ditty Putnam County, Tennessee and M. Jones age 37 PO Ditty, Tenn. who being duly sworn declares in relation to said case as follows: Affiants personally acquainted with applicant and know him to require the constant aid and attention of another person for the purpose of feeding him his food, has to be cut up for him on his plate and carried to his room. Also has to be dressed and undressed by his nurse from five to ten times in twenty four hours and also has to be helped up to his chamber very often as he has no control of his bowels and affiants further states that said applicant has not been over ten steps from his room in near two years. The above affidavit was written by affiant M. Jones, and in the presents of Ira. I. Howell. And is written from personal knowledge, no written or printed statement, or recital prepared or dictated by any other person. All of whom I certify to be respectable and entitle credit and who being sworn by me according to law say that they are personally acquainted with Ammond D. Jones of Co. A., 5th Tenn. Cavalry and has been for 40 years and know that he is totally helpless and has been for ten or eleven years requiring the aid of an attendant all the time and for the last three years requiring the aid of an assistant. They further state that the pension of $17.00 per month is inadequate to his disabilities. Said soldier had an application for increase rejected. They state that they don't think he had justice done in the matter and ask that the case be reopened. They further state that they have no interest in the prosecution claim in any manner whatever.
