State of Tennessee, County of White On this 5 day of June, A. D. one thousand, nine hundred and 12, personally appeared before me, R.D. Floyd Notary Public within and for the county and State aforesaid, Burtis Saylors who, being duly sworn according to the law, declares that he is 69 years of age, and a resident of Bakers X Roads, county of White, State of Tennessee, and that he is the identical person who was enrolled at New Ark, White Co. Tennessee under the name of Burtis Saylors on the 11 day of October, 1864, as a Private in Company K Private First Tennessee Mounted Infantry Vol. In the service of the United States, in the Civil war, and was honorably discharged at Nashville, Tennessee, on the 22 day of July 1865. That he was not employed in the military or naval service of he United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, 6 feet 2 inches; complexion Fair; color of eyes, yellow; color of hair, Red; and his occupation was Farmer; that he was born 30 day of August 1842 in White County Tennessee. That his several places of residence since leaving the service have been as follows: I have not lived no where else only in White County. That he is a pensioner under certificate No. 595238 That he has not applied for pension under original No. May 11, 1912 That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912 He hereby appoints with full power of substitution and revocation. July 22, 1865 From Oct. 1, 1864 to July 22, 1865, he held the rank of Pvt. And during that period the rolls show him present Name appears only on Co. M.I. and M. O. roll. The medical records show him treated as follows As Burtis W. Saylors, Pvt. Co. K. 1 Tenn. M. I., May 24 to June 26, 65, Acute Diarrhea, returned to duty; disposition also shown "discharged from service May 22, 65, mustered out." Nothing additional found B. W. Saylors Pvt. Co. K. (Capt. Osgathorp's) 28 Tenn. Vol., C. S. A. Enlisted Sept. 22/62 at Bunker Hill Tenn by a Sergt. As Esquire and Enrolling Officer, for 3 years and is reported on roll of Co. for Sept. and Oct./62 (without date) "absent missing since the battle Jan 2/63." Jan and Feb./63 - not on file, and sub. Rolls, name dropped Name Burtis Saylors not _____. Roll of Co. __ (above Reg't) for July and Aug./63 - first and only roll on which name is_____ shows: B. W. Saylors enlisted Sept. 22/62 at White Co. __ Saylors for 36 mos., present. No record of disability found. State of Tennessee, White County, To any regular Ordained Minister of the Gospel having the care of souls, or to any Justice of the Peace, or to any Judge or Chancellor of said State. Bond and security having been given according to the law, you are hereby authorized to solomnize the Rites of Matrimony between B. W. D. 1915, personally appeared before me, a Notary Public within and for the County and State aforesaid Harriet Saylors, aged 66 years, a resident of Sparta, County of White, State of Tennessee, who, being duly sworn according to the law, makes the following declaration for the purpose of being placed on the pension roll of the United States under the provision of the Act of April 19, 1908. That she is the widow of Burtis W. Saylors, who was enrolled under the name of Burtis W. Saylors at _____ on the 11th day of October 1864 as a Pvt. In Co. K 1st Tenn. Mtd. Inft. And served at least ninety days during the War of the Rebellion in the service of the United States and was honorably discharged on the 22 day of July, 1865, and who died at Sparta R.R. #7, on the 10th day of January, 1915. That she was married under the name of Harriet J. Southard to said soldier at her father's home on the 25th day of February, 1866 by ___ington McConnell; that there was no legal barrier to the marriage; that she had not been previously married; that the soldier had not been previously married. That she has not remarried since the death of said soldier. That she has not heretofore applied for pension. She hereby appoints J. B. Cralle Co.
