     Ethel Mae TRAVIS - Nov. 08, 1911 - May 19, 1928                                       
                                           
I received a copy of Ethel Mae TRAVIS death certificate obtained from
the Texas Department of Health, Bureau of Vital Statistic's, date of my
Issue Dec. 31, 1998. 
The death record's stamped numbered is Reg. # 349987, on form D, with
a hand written Registered # 1129 in upper right hand corner of this 
paper. With a number of E731709 in the lower left hand corner of this 
paper. Certification of Vital Record from the State of Texas.
     
     This record states the following information for Ethel Mae Travis:
     
Place of Death is Dallas County, City of Dallas, in Parkland Hospital.
Ethel's residence listed as Garland, Texas.
States that Ethel was Female, White & Single.
That her DOB was Nov. 08, 1911 and that she was 16 yrs, 6 mo.'s, & 
11 ds. old at the time of her death.
Occupation is listed as "School girl".
Birthplace is listed as OKLAHOMA.
Father is listed as Joe TRAVIS.
Birthplace of father is listed as TENNESSEE.
Maiden Name of mother is listed as Ellen BUTLER.
Birthplace of mother is listed as TENNESSEE.
Signed as true information by Mrs. M.L. BUTLER of Wylie, Texas.

(This was probably Mrs. Martin Luther BUTLER otherwise known as
Minnie BUTLER who was the wife of Martin Luther BUTLER).

This also states that Ethel's Date of Death was May 19, 1928 and it is 
noted here that she had been under the care of J. B. Drainer, MD. since
May 17, 1928.
It also shows that Ethel's Cause of Death is listed as" MENENGITIS "
and that an autopsy had been performed after her death.

Ethel's parents were Ellen (BUTLER) TRAVIS who was born abt. 1875 in
Grainger Co., TN. Her father Joseph N. Travis was born abt. 1875 in 
Jefferson Co., TX. they had lived in Fannin CO. for awhile but moved on,
to Wapanucka, OK. 

Velma's grandparents were David Tate Travis & Nancy Adaline (Pierce) 
TRAVIS of Jefferson Co.,TN. and Calvin M. Butler & Ellen Virginia 
(Roland) BUTLER of Grainger Co., TN., Fannin Co., TX.
